基于数据挖掘分析古籍针灸治疗面瘫的腧穴应用规律

Acupoints Selection in Treating Facial Paralysis Recorded in Ancient Books Based on Data Mining

目的:运用数据挖掘技术分析古籍中针灸治疗面瘫的腧穴应用规律。方法:收集第5版《中华医典》针灸推拿分部中所收录的53部医籍中涉及针灸治疗面瘫的相关条文,建立“针灸治疗面瘫的古籍数据库”,运用数据挖掘技术中的聚类分析及关联分析法则分析古代针灸治疗面瘫用穴特点及规律。结果:共纳入条文298条,针灸处方103条,涉及腧穴53个,总频次448次,腧穴选用以地仓、颊车、承浆、水沟为主,经脉以阳明经为主,部位以头面颈项部最频繁,特定穴应用中以交会穴及五输穴的荥穴为主;聚类分析得出4个有效聚类群:上关-下关,翳风-通天-冲阳-腹通谷,偏历-行间-内庭-二间,合谷-水沟;关联分析得出常用穴组共8对。结论:古代针灸治疗面瘫尤其重视地仓-颊车-听会,水沟-合谷-承浆与其他腧穴的灵活配伍,体现“经脉所过,主治所及”的取穴原则,通过配伍丰富腧穴的多效性,且常见配伍方式主要为局部取穴结合远道取穴。

Objective:To analyze the rules of acupoints selection in the treatment of facial paralysis recorded in ancient books based on data mining.Methods:The relative data were collected from lines recorded in the 53 ancient books which were included in the 5th edition of Chinese Medical Code,so as to establish the database of acupuncture and moxibustion in treating facial paralysis in ancient books.Relevance rule and cluster analysis methods in data mining technology were applied to analyze the rules of acupoints selection in the treatment of facial paralysis.Results:Totally 298 lines were collected,involving 103 pieces of prescription,53 acupoints with a total frequency of 448.The results indicated that the leading selection of acupoints were ST4,ST6,RN24 and DU26.The meridians were focused on the Yangming meridians,especially the acupoints on the Stomach Meridian of foot Yangming.The most commonly used acupoints were located on head,face and cervical parts.The crossing points and spring points were mostly selected among the special acupoints.In addition,four efficient clusters were acquired from cluster analysis,i.e.GB3-ST7,SJ17-BL7-ST42-KI20,LI6-LR2-ST44-LI2 and DU26-LI4.Eight acupoint combinations were obtained from association rules analysis.Conclusion:It is concluded that ancient acupuncture emphasized the flexible compatibility of ST4-ST6-GB2 and DU26-RN24-LI4 with other acupoints.Distal-proximal acupoint combination is the most frequent method and the compatibility of acupoints can increase the pleiotropic effect.The principle of selecting acupoints along the meridians can also be seen.
