超微针刀治疗腰椎间盘突出症对腰椎功能的改善作用分析

Function Improvement of Lumbar Vertebra Treated by Superfine Needle Knife in LDH Patients

目的:探讨超微针刀治疗腰椎间盘突出症(LDH)对患者腰椎功能的改善作用。方法:选取2016年4月—2018年10月本院收治的LDH患者112例,采用随机数字表法将患者分为实验组和对照组,各56例。对照组采用温针灸治疗,实验组采用超微针刀治疗,两组均连续治疗2周。比较两组治疗后临床疗效、首次显效时间、治疗前后腰腿疼程度、腰椎功能、下肢功能及腰椎屈伸最大肌力,记录不良反应发生情况。结果:实验组总有效率明显高于对照组(P<0.05);实验组首次显效时间明显早于对照组(P<0.01);与治疗前比较,治疗后两组患者腰腿部VAS评分及ODI均明显降低,且实验组明显低于对照组(P<0.01);两组直腿抬高角度均明显增大,且实验组明显大于对照组(P<0.01);与治疗前比较,治疗后两组患者腰椎前屈、后伸、左右侧屈最大肌力均明显升高,且实验组明显高于对照组(P<0.01);两组总不良反应发生率无明显差异(P>0.05)。结论:超微针刀治疗LDH可显著缓解患者腰腿部疼痛,改善腰椎功能及下肢功能障碍,临床疗效较好,安全有效,更适合在基层医院临床推广应用。

Objective:To explore the improvement effect of superfine needle knife on function of lumbar vertebra in patients with lumbar disc herniation(LDH).Methods:112 LDH cases were randomly divided into the observation group and the control group,with 56 cases in each group.The control group was treated with needling-warming moxibustion,and the observation group was treated with superfine needle knife.The two groups were treated continuously for two weeks.The clinical efficacy,the first time to response,the degree of lumbar and leg pain,the function of lumbar vertebra,the function of lower limb,the maximal muscle strength of lumbar flexion and extension were compared before and after the treatment.The adverse reactions were recorded.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The first time to response was significantly earlier in the observation group than that in the control group(P<0.01).The VAS score and ODI score of lumbar and leg decreased greatly after the treatment in the two groups,and the decreases were more significant in the observation group than those in the control group(P<0.01).The straight leg elevation angles increased significantly in the two groups after the treatment,and the increases were more significant in the observation group(P<0.01).The maximal muscle strength of lumbar antexion,posterior extension and lateral flexion increased significantly in the two groups after the treatment,and the changes of the observation group were more obvious(P<0.01).There was no statistical difference in the incidence of total adverse reactions between the two groups(P>0.05).Conclusion:Superfine needle knife can significantly relieve the pain degree of lumbar and leg in the treatment of LDH,it can improve the function of lumbar vertebra and lower limb.The therapy is safe and effective,and it's more suitable for clinical promotion in primary hospitals.
