摘要
坏消息告知是医患沟通中的重要内容。坏消息具有主观性、相对性、广泛性、消极性四个特征,中晚期癌症的特点及传统观念的影响,是中晚期癌症治疗中坏消息告知困难的重要原因。通过对实践中临床工作的调研、观察与访谈,阐释医务人员对中晚期癌症患者坏消息告知过程中来自家属转告、医学局限性、隐瞒病情及跨文化告知方面的困境,并进一步提出应对措施,以期通过优化医患之间坏消息告知,充分发挥医学人文在给予中晚期癌症患者支持、降低隐瞒风险以及弥补跨文化差异中的重要作用。
Breaking bad news is an important part of doctor-patient communication.Bad news has four characteristics:subjectivity,relativity,universality and negativity.The particularity of middle and advanced cancer and the influence of traditional concepts are the important reasons of the difficulty in breaking bad news for the treatment of middle and advanced cancer.Through the investigation,observation and interview of clinical work in practice,this paper explained difficulties,which came from family informing,medical limitation,concealing condition and cross-cultural notifying in the process of breaking bad news that happened between doctors and patients with middle and advanced cancer.And further,this paper put forward to countermeasures to optimize breaking bad news between doctors and patients,to bring medical humanities into full play in rendering support for patients with middle and advanced cancer,to reduce the risk of concealment and making up for cross-cultural differences.
