钢板与交锁髓内钉内固定治疗股骨干骨折的疗效比较

Comparison of the effects of plate and interlocking intramedullary nailing in the treatment of femoral shaft fractures

目的探讨钢板与交锁髓内钉内固定治疗股骨干骨折的临床疗效。方法对110例股骨干骨折患者分别采用闭合复位交锁髓内钉内固定(交锁髓内钉组,45例)和切开复位钢板内固定(钢板组,65例)治疗。比较两组患者术中出血量、手术时间、骨折愈合时间、并发症及术后12个月HSS评分。结果患者均获得随访,时间12~36个月。术中出血量、骨折愈合时间、并发症发生率及术后12个月HSS评分交锁髓内钉组均少(优)于钢板组(P<0.05),手术时间两组比较差异无统计学意义(P>0.05)。结论采用交锁髓内钉内固定治疗股骨干骨折,失血少、愈合时间快、功能恢复满意。

Objective To investigate the clinical efficacy of plate and interlocking intramedullary nail in the treatment of femoral shaft fractures.Methods The 110 cases of femoral shaft fractures were treated with closed reduction and interlocking intramedullary nail fixation(interlocking intramedullary nail group,45 cases)and open reduction and plate internal fixation(plate group,65 cases).The intraoperative bleeding volume,operative time,fracture healing time,complications,and HSS score at 12 months postoperation were compared between two groups.Results All patients were followed up for 12~36 months.The intraoperative blood loss,fracture healing time,complications incidence rate and HSS score of 12 months postoperation in the interlocking intramedullary nail group were less(better)than those in the plate group(P<0.05),but there was no significant difference in the operation time between the two groups(P>0.05).Conclusions Interlocking intramedullary nailing for the treatment of femoral shaft fractures has less blood loss,faster healing time and satisfactory functional recovery.
