山东省综合医院肿瘤专业医务人员临终关怀现状调查及影响因素分析

Investigation on the status of hospice care and its influencing factors of cancer professional medical staff in General Hospital of Shandong Province

目的了解山东省综合医院肿瘤专业医务人员临终关怀的态度、知识、行为现状及其影响因素,为综合医院开展临终关怀工作提供理论依据。方法采用自行设计的《医务人员临终关怀调查表》,对山东省31家综合医院内肿瘤科的医务人员进行随机整群调查,共得到1037份有效问卷。采用SPSS22.0版进行统计分析。结果1037名医务人员临终关怀得分平均为3.99,标准分79.8,接近良好水平;临终关怀态度得分平均4.01,标准分80.2,达良好水平;临终关怀知识得分平均3.88,标准分77.6,为及格水平;临终关怀行为得分平均3.92,标准分78.4,为及格水平;临终关怀评价得分4.20,标准分84,达良好水平。医院级别、性别、年龄、学历、工作年限、职业、行政职务、月收入、宗教信仰、家庭形态不同的医务人员,临终关怀得分有差异(P﹤0.05)。结论山东省综合医院肿瘤专业医务人员的临终关怀总体水平较高,对临终关怀的态度及评价较好,但临终关怀知识仍欠缺,临终关怀行为需加强。对二级医院、年轻医务人员、学历较低、低收入人群者、护士、科主任等应分别加强临终关怀意识的培养,临终关怀知识的培训,临终关怀行为的规范等。

Objective:To understand the attitude,knowledge and behavior and influencing factors of hospice care of cancer professional medical staff in the general hospital of Shandong province,and provide theoretical basis for hospice care in general hospitals.Methods A randomized questionnaire survey was conducted among the cancer professional medical staff of 31 general hospitals in Shandong Province by using the self-designed"Medical staff hospice questionnaire".A total of 1037 valid questionnaires were obtained.SPSS22.0 was used for statistical analysis.Results Among 1037 medical staff,the average score of hospice care is 3.99,the standard points is 79.8,close to the good level.The average score of the attitude of hospice care is 4.01,the standard points 80.2,up to good level.The average score of the knowledge of hospice care is 3.88,the standard points is 77.6,up to passing level.The average score of the behavior of hospice care is 3.92,the standard points is 78.4,up to passing level.The average score of the evaluation of hospice care is 4.20,the standard points is 84,up to good level.There are significant differences in the scores of hospice care in the level of hospital,gender,age,education,working life,occupation,administrative function,monthly income,religious belief and family form(P<0.05).Conclusions The overall level of hospice care is high,the attitude and the evaluation of hospice care is good of the cancer professional medical staff in General Hospital of Shandong Province,but knowledge of hospice care is still lacking,the behavior of hospice care needs to be strengthened.We should strengthen the cultivation of hospice care awareness,the training of hospice care knowledge and the specification of hospice care behavior of secondary hospitals,young medical staffs,lower education people,low-income people,nurses,directors.
