基于风险管理的护理干预对乳腺癌患者行经外周静脉置入中心静脉导管并发症的预防效果

Effect of nursing intervention based on risk management in prevention of complications caused by peripherally inserted central catheter in breast cancer patients

目的探讨基于风险管理的护理干预对乳腺癌患者经外周静脉置入中心静脉导管(PICC)并发症的预防效果。方法将2016年3月—2018年2月接受常规PICC护理的50例患者设为对照组,2018年3月—2019年6月接受PICC风险管理护理干预的50例患者设为研究组。比较2组PICC置管操作情况和相关并发症发生率。结果2组一次性置管成功率比较无显著差异(P>0.05);研究组一次性穿刺成功率显著高于对照组,置管操作时间显著短于对照组(P<0.05)。2组静脉血栓、导管堵塞发生率比较无显著差异(P<0.05),但研究组静脉炎、渗血、感染发生率均显著低于对照组(P<0.05)。结论开展基于风险管理的护理干预能明显提高乳腺癌术后PICC置管操作质量,有效预防静脉炎、渗血、感染等并发症的发生。

Objective To explore effect of nursing intervention based on risk management in prevention of complications caused by peripherally inserted central catheter(PICC)in breast cancer patients.Methods A total of 100 patients with breast cancer who received PICC routine care from March 2016 to February 2018 in our hospital were selected as control group,and those with PICC risk management from March 2018 to June 2019 in our hospital were selected as study group.The operation conditions of PICC and the incidence of complications were compared between the two groups.Results There was no significant difference in success rate of one-time catheterization between the two groups(P>0.05).The success rate of one-time puncture was higher in the study group and catheterization time was shorter than that in the control group(P<0.05).There was no significant difference between the two groups in the incidence of venous thrombosis and catheter blockage(P>0.05).The study group had lower incidence of phlebitis,bleeding and infection caused by PICC catheterization than that of the control group(P<0.05).Conclusion Nursing intervention based on risk management can significantly improve the quality of PICC catheterization after breast cancer operation,and effectively prevent phlebitis,bleeding,infection and other complications.
