体素内不相干运动扩散加权成像在前列腺癌及前列腺增生鉴别诊断中的应用 被引量:6

摘要 目的探讨体素内不相干运动扩散加权成像(IVIM-DWI)在前列腺癌与前列腺增生鉴别诊断中的应用价值。方法回顾性分析2016年1月至2018年1月该院收治的25例前列腺癌(前列腺癌组)、20例良性前列腺增生(前列腺增生组)患者的IVIM-DWI参数;比较2组患者IVIM-DWI参数真实的扩散系数(D)、伪扩散系数(D*)、灌注分数(f)的差异,并通过受试者工作特性曲线评估其诊断效能。结果前列腺癌组患者D、f值明显低于前列腺增生组,D*值明显高于前列腺增生组,差异均有统计学意义(P<0.05),各参数的诊断效能以D、f值较高。结论IVIM-DWI可有效鉴别诊断前列腺癌与前列腺增生,其中D、f值更具有优势。 Objective To investigate the application value of intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)in the differential diagnosis of prostate cancer and benign prostatic hyperplasia.Methods The IVIM-DWI parameters of 25 patients with prostatic cancer(prostatic cancer group)and 20 patients with benign prostatic hyperplasia(prostatic hyperplasia group)in the hospital from January 2016 to January 2018 were retrospectively analyzed.And the differences in the true diffusion coefficient(D),false diffusion coefficient(D*)and perfusion fraction(f)were compared between the two groups and their diagnostic efficiency was assessed by using the receiver operating characteristic(ROC)curve.Results The values of D and f in prostate cancer group were significantly lower than those in the prostatic hyperplasia group,and the value of D*was significantly higher than that in the prostatic hyperplasia group,and the differences were statistically significant(P<0.05).The diagnostic efficiency of various parameters was higher with the values of D and f.Conclusion IVIM-DWI can effectively identify prostate cancer and prostatic hyperplasia,in which the D and f values have more advantages.
