虚拟现实技术物理疗法改善脑卒中患者失眠的效果研究 被引量:11

摘要 目的探讨虚拟现实(VR)技术物理疗法改善脑卒中患者失眠的效果。方法采用方便抽样方法,通过随机数字表法将2017年4月至2018年12月该院收治的脑卒中后失眠患者50例分为研究组(VR组)和对照组(药物组),每组25例。VR组运用VR技术物理疗法,药物组给予短效苯二氮类药物口服,均持续观察2周,评定2组患者入院时、出院时入睡非快速眼动(NREM)潜伏期,觉醒次数,以及汉密尔顿焦虑量表(HAMA)、简易智力状态检查量表(MMSE)评分等,并进行统计分析。比较2组患者入睡时间和睡眠情况,分析VR技术物理疗法在改善脑卒中患者失眠中的效果,以及VR技术物理疗法对脑卒中失眠患者生活质量、认知功能的影响。结果VR组患者出院时入睡NREM潜伏期明显低于入院时,差异有统计学意义(P<0.05),VR组患者入院时及出院时的入睡NREM潜伏期、觉醒次数与药物组比较,差异均无统计学意义(P>0.05),VR组患者出院时MMSE、HAMA评分与药物组比较,差异均有统计学意义(P<0.05)。结论VR技术物理疗法改善脑卒中患者失眠效果显著,且对脑卒中失眠患者生活质量、认知功能无不良影响,可作为临床优化治疗的参考方案。 Objective To explore the effect of virtual reality(VR)technology physical therapy on improving insomnia in stroke patients.Methods The convenient sampling method was adopted.Fifty patients with insomnia after stroke treated in the hospital from April 2017 to December 2018 were divided into the study group(VR group)and control group(drug group)by using the random number table,25 cases in each group.The VR group used the VR technique physical therapy,and the drug group was orally given the short-acting benzodiazepines.All cases were continuously observed for 2 weeks.The latency to non-rapid eye movement(NREM),number of awakenings,scores of the Hamilton Anxiety Scale(HAMA)and the Simple Intelligence Status Examination Scale(MMSE)at admission and discharge were assessed in the two groups and statistically analyzed.The sleep time and sleep conditions were compared between the two groups.The effect of VR technology physical therapy on improving insomnia in stroke patients and its influence on the quality of life and cognitive function were analyzed.Results The latency of sleep NREM at discharge in the VR group was significantly lower than that at admission,and the difference was statistically significant(P<0.05).The latency of sleep NREM and the number of awakenings at admission and discharge had no statistical difference between the VR group and drug group(P>0.05).The MMSE score and HAMA score at discharge had statistical difference between the VR group and drug group(P<0.05).Conclusion The VR technology physical therapy has significant effect for improving the insomnia in stroke patients,and has no adverse effects on their quality of life and cognitive function,which can serve as a reference scheme for clinical optimization treatment.
