基于功能法卫生费用核算的吉林省恶性肿瘤治疗费用分析

Analysis on the Curative Care Expenditure of Malignant Tumor in Jilin Province Based on SHA2011

目的:分析吉林省恶性肿瘤的治疗费用规模、服务功能和受益人群,为制定恶性肿瘤的防控策略和合理配置卫生资源提供依据。方法:以功能法卫生费用核算体系(SHA2011)为理论框架,对吉林省恶性肿瘤的治疗费用进行核算分析。结果:2017年吉林省恶性肿瘤治疗费用为561371.10万元,女性治疗费用为292428.36万元,男性治疗费用为268942.77万元;15-59岁人群治疗费用最高,占全人群治疗费用的52.18%;吉林省恶性肿瘤治疗费用居于前3位的依次是消化器官恶性肿瘤、呼吸和胸腔内器官恶性肿瘤、乳房恶性肿瘤;次均治疗费用居于前3位的疾病为独立(原发)多个部位的恶性肿瘤,眼、脑和中枢神经系统其他部位的恶性肿瘤,消化器官恶性肿瘤。结论:吉林省恶性肿瘤治疗费用总量较大,女性较男性治疗费用高;在疾病治疗费用构成中,消化系统恶性肿瘤和呼系统恶性肿瘤治疗费用居于前2位;重点疾病次均治疗费用高,患者疾病负担重。

Objective: To analyze the curative care expenditure, service function and beneficiary population of malignant tumors in Jilin province, and to provide evidence for formulating prevention and control strategies of malignant tumors and rational allocation of health resources. Methods: The System of Health Accounts 2011(SHA2011) was used to analyze the curative care expenditure of malignant tumor in Jilin province. Results: In 2017, the total curative care expenditure of malignant tumors in Jilin province was 5.614 billion yuan, the curative care expenditure of female was 2.924 billion yuan, and the curative care expenditure of male was 2.690 billion yuan. The curative care expenditure was the highest in the 15-59 years old population, accounting for 52.18% of the total curative care expenditure. The top three of curative care expenditure of malignant tumors in Jilin province were malignant tumors of the digestive organs, malignant tumors of the respiratory and intrathoracic organs, malignant tumors of the breasts. The top three of curative care expenditure per treatment were malignant tumors in multiple sites independently(primary), malignant tumors in the eyes, brain and other parts of the central nervous system, and malignant tumors in the digestive organs. Conclusion: The scale of malignant tumor treatment expenses in Jilin province is large, and the treatment cost of female is higher than that of male. In the cost of disease treatment, more concentrated in the curative care expenditure of the digestive system diseases and respiratory system diseases. The average treatment cost of priority diseases is high, and the burden of disease is heavy.
