p53和CD56在甲状腺微小癌合并转移的表达及其相关性研究 被引量:2

摘要 目的探究p53和CD56在甲状腺微小癌(TMC)合并转移患者体内表达情况,并分析其相关性。方法选择2017年1月至2018年12月于该院接受治疗的TMC患者154例设为观察组,按淋巴结是否转移细分为合并转移和非转移,另选取同时间段同年龄段非甲状腺癌手术患者作为对照组,比较各组间之间p53和CD56表达差异,分析上述两个指标与TMC病理参数间的关系。结果两组p53、CD56和CK19指标表达水平比较差异有统计学意义(P<0.05);p53表达与淋巴结转移、TNM分期、组织分化程度密切相关(P<0.05),CD56表达仅与淋巴结转移呈明显相关性(P<0.05);p53+CD56的诊断TMC发生具较高准确率(96.57%),p53、CD56单独应用时准确率明显低于联合检测(P<0.05),上述3种方法检测TMC转移准确率均不超过70%。结论在TMC合并转移病例中p53多阳性表达,CD56阴性表达;在非转移TMC中p53更趋于阴性表达,CD56阴性表达比例更低,两者均与淋巴结转移明显相关,可辅助诊断TMC的发生和转移。 Objective To investigate the expression of p53 and CD56 in patients with thyroid microcarcinoma and metastasis,and to analyze their correlation.Methods A total of 154 patients with thyroid microcarcinoma(TMC)who were treated in our department from January 2017 to December 2018 were enrolled in the observation group.The lymph node was subdivided into a combined metastasis group and a non-metastasis group.The patients with non-thyroid cancer who were in the same age group were served as the control group.The differences of p53 and CD56 expression between the groups were compared.The relationship between the above two indexes and the pathological parameters of TMC was analyzed.Results The expression levels of p53,CD56 and CK19 were significantly different between two groups,the difference was statistically significant(P<0.05).p53 expression was closely related to lymph node metastasis,TNM stage and tissue differentiation(P<0.05).CD56 expression was only correlated with lymph node metastasis(P<0.05).The diagnosis of p53+CD56 was more accurate(96.57%).The accuracy of p53 and CD56 alone was significantly lower than that of the combined detection(P<0.05).The accuracy of the above three methods for detecting TMC transfer did not exceed 70%.Conclusion In TMC with metastasis patients,p53 was more positively expressed and CD56 was negatively expressed.In TMC with non-metastasis patients,p53 is more negatively expressed and CD56 negative expression is lower.Both of which are significantly associated with lymph node metastasis,and can assist in the diagnosis of the occurrence and metastasis of thyroid microcarcinoma.
