丙种球蛋白联合糖皮质激素对过敏性紫癜肾炎患儿尿蛋白及免疫功能的影响

Effects of gamma globulin combined with glucocorticoids on urinary protein and immune function in children with Henoch-Schonlein purpura nephritis

目的分析丙种球蛋白联合糖皮质激素对过敏性紫癜肾炎(HSPN)患儿尿蛋白及免疫功能的影响。方法选取2017年1月至2018年7月我院HSPN患儿40例,随机数字表法分为观察组(20例)与对照组(20例)。2组均给予抗过敏、口服钙剂及营养支持治疗。另外,对照组接受糖皮质激素治疗,观察组接受丙种球蛋白+糖皮质激素治疗。观察对比2组治疗效果、治疗前后尿蛋白定量(24 h)、免疫功能[血浆T细胞亚群(CD4+、CD8+)]。结果观察组总有效率90%高于对照组(75%)(P<0.05);治疗1个月后,观察组CD8+水平低于对照组,且CD4+水平高于对照组(P<0.05);治疗1个月后,观察组尿蛋白定量(24 h)水平低于对照组(P<0.05)。结论丙种球蛋白联合糖皮质激素治疗HSPN的疗效显著,可降低尿蛋白定量(24 h),有效改善免疫功能。

Objective To investigate the effects of gamma globulin combined with glucocorticoids on urine protein and immune function in children with Henoch-Schonlein purpura nephritis(HSPN).Methods A total of 40 HSPN children in our hospital between January 2017 and July 2018 were enrolled,and by random number table method,divided into the study group(n=20)and control group(n=20).Both groups were treated with antianaphylaxis,oral calcium and nutritional support.Besides,the control group received glucocorticoids,and the study group received gamma globulin+glucocorticoids.The two groups were compared for therapeutic effects,24 h urine protein before and after treatment,and immune function[plasma T cell subsets(CD4+,CD8+)].Results The total effectiveness rate in the study group(90%)was higher than that in the control group(75%,P<0.05).At 1 month after treatment,the study group showed lower CD8+level,higher CD4+level,and reduced 24 h urine protein,compared with the control group(P<0.05).Conclusion The combination of gamma globulin and glucocorticoids in the treatment of children with HSPN lead to significant therapeutic effects in effectively reducing 24 h urine protein and improving immune function.
