抗环瓜氨酸肽抗体、类风湿因子、C-反应蛋白、红细胞沉降率在类风湿性关节炎中的临床应用 被引量:28

Clinical application of anti-cyclic citrullinated peptide antibodies, rheumatoid factor, C-reactive protein and erythrocyte sedimentation rate in rheumatoid arthritis

摘要 目的评价联合检测抗环瓜氨酸肽抗体(抗CCP抗体)及类风湿因子(RF)在类风湿性关节炎(RA)临床诊断中的价值,并探讨抗CCP抗体、RF、C-反应蛋白(CRP)、红细胞沉降率(ESR)等实验室指标在类风湿性关节炎病情活动评估中的应用。方法选择80例正常体检者,83例RA患者,分别检测抗CCP抗体、RF、CRP、ESR,并根据临床检查与CRP计算RA患者的DAS28值。结果抗CCP抗体和RF对RA诊断的灵敏度分别为85.54%、75.90%,特异度分别为82.50%、78.75%。RF、CRP、ESR在RA低活动组与高活动组之间差异有统计学意义(P<0.05)。结论抗CCP抗体对RA诊断具有良好的应用价值,与RF联合检测可以提高灵敏度、特异性。类风湿性关节炎患者RF、CRP、ESR水平对RA的病情活动度评估有重要意义。 Objective To evaluate the combined detection value of anti-cyclic citrullinated peptide antibodies(anti-CCP) and rheumatoid factor(RF) in the clinical diagnosis of rheumatoid arthritis(RA), and to explore the role of anti-CCP, RF, C-reactive protein(CRP), erythrocyte sedimentation rate(ESR) and other laboratory markers in assessing disease activity in rheumatoid arthritis. Methods Eighty normal volunteers and eighty-three patients with RA were selected for the detection of anti-CCP antibody, RF, CRP, ESR. Calculation of DAS28 values for patients with RA were performed according to clinical examination and CRP. Results The diagnostic sensitivity of anti-CCP antibody and RF to RA was 85.54% and 75.90%, and the specificities were 82.50% and 78.75%. The differences of RF, CRP and ESR between low RA activity group and high RA activity group were statistically significant(P<0.05). Conclusion Anti-CCP has good application value to the diagnosis of RA. Combined detection of anti-CCP and RF has a higher sensitivity and specificity. RF, CRP, ESR have important significance to evaluate disease activity of RA.
