老年2型糖尿病患者合并骨质疏松的相关危险因素分析 被引量:9

摘要 目的分析老年2型糖尿病患者合并骨质疏松的相关危险因素。方法收集124例年龄≥65岁的2型糖尿病患者病历资料。根据骨密度检测结果,分为骨质正常组和骨密度降低组(包括骨量减少组和骨质疏松组),其中骨质正常者40例,骨密度降低组84例。对这两组患者的性别、糖尿病病程、体质指数(BMI)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血钙(Ca)、血磷(P)、血尿酸(UA)、总胆固醇(TC)、25-羟基维生素D[25-(OH)D3]指标进行比较分析。结果老年2型糖尿病患者合并骨密度降低组的女性比例明显高于男性比例(P=0.022)。多因素logistic回归分析得出:糖尿病病程、BMI、HbA1c、P、25-(OH)D3均是老年2型糖尿病患者合并骨质疏松的独立危险因素(P<0.05)。结论长糖尿病病程、低BMI、高HbA1c、高P、低25-(OH)D3是老年2型糖尿病患者合并骨质疏松的危险因素。 Objective To analyze the risk factors of elderly patients with type 2 diabetes associated with osteoporosis.Methods A total of 124 patients with T2 DM aged 65 years or older were enrolled.On the basis of bone mineral density data,the patients were divided into the normal bone mass group(n=40)and low bone mass group(osteopenia group and osteoporosis group(n=84).The gender,diabetes duration,body mass index(BMI),fasting blood glucose(FBG),hemoglobin A1 c(HbA1c),serum calcium(Ca),blood phosphorus(P),blood uric acid(UA),total cholesterol(TC),and 25-hydroxyvitamin D[25-(OH)D3]among the two groups were compared and analyzed.Results The proportion low bone mass in female elderlly with T2 DM was significantly higher than that of male group(P=0.022).The duration of diabetes,BMI,HbA1c,P,and 25-(OH)D3 were found as key risk factors of elderly patients with T2 DM associated with osteoporosis after multivariate logistic regression analysis.Conclusion The duration of diabetes,low BMI,high HbA1c,high P,25-(OH)D3 are the risk factors of teoporosis in elderly patients with T2 DM.
