摘要 目的分析CT与MRI对隐匿性胫骨平台骨折的诊断价值。方法选取2018年1月至12月我院收治的疑似隐匿性胫骨平台骨折患者86例,所有患者均行CT及MRI检查。分析两种检查方法在隐匿性胫骨平台骨折中的诊断价值,比较两种方法对不同类型隐匿性胫骨平台骨折的检出率。结果 MRI诊断隐匿性胫骨平台骨折的灵敏度及准确度均高于CT (P<0.05);两种方法诊断隐匿性胫骨平台骨折的特异度比较,差异无统计学意义(P>0.05)。MRI对骨小梁骨折的检出率高于CT,对骨皮质骨折的检出率低于CT (P<0.05)。结论 CT与MRI均可诊断隐匿性胫骨平台骨折,但MRI具有更高的灵敏度及准确度,两种检查方法联合使用有利于减少漏诊,可获得更高的诊断价值,为临床诊断及治疗提供影像学参考依据。 Objective To analyze the diagnostic value of CT and MRI for occult tibial plateau fractures. Methods 86 patients with suspected occult tibial plateau fractures admitted to our hospital from January 2018 to December 2018 were selected. All patients underwent CT and MRI examinations. The value of CT and MRI in the diagnosis of occult tibial plateau fractures were analyzed, and the detection rates for different types of occult tibial plateau fractures of CT and MRI were compared. Results The sensitivity and accuracy of MRI in the diagnosis of occult tibial plateau fractures were significantly higher than those of CT(P<0.05), while no statistical difference was found between CT and MRI in the specificity in the diagnosis of occult tibial plateau fracture(P>0.05). The detection rate for trabecular fracture of MRI was significantly higher than that of CT, and the detection rate for cortical fracture of MRI was significantly was significantly lower than that of CT(P<0.05). Conclusions Both CT and MRI can diagnose the occult tibial plateau fractures, but MRI has higher sensitivity and accuracy. The combination of the two methods can reduce the missed diagnosis, obtain higher diagnostic value, and provide imaging reference for clinical diagnosis and treatment.
