常规超声及超声造影检查在肾脏占位性病变中的诊断价值分析

Analysis on the Diagnostic Value of Conventional Ultrasound and Contrast-Enhanced Ultrasonography Examinations for Renal Occupying Lesions

目的探讨常规超声及超声造影在肾脏占位性病变中的诊断价值。方法选择2016年3月至2019年3月就诊于我院的肾脏占位性病变患者78例,均接受常规超声及超声造影检查。以病理诊断结果为金标准,分析常规超声与超声造影诊断肾脏占位性病变的价值和主要肾脏占位性病变不同时相病变的超声造影增强程度。结果 78例患者的病理诊断结果显示,恶性病变37例,良性病变41例;超声造影诊断肾脏占位性病变的敏感性、特异性、准确性分别为94.59%、92.68%、93.59%,高于常规超声的78.38%、65.85%、71.79%(P均<0.05)。肾透明细胞癌髓质期、皮质期超声造影增强强度高于平滑肌脂肪瘤(P均<0.05);肾透明细胞癌消退期增强强度与平滑肌脂肪瘤比较,差异无统计学意义(P>0.05)。结论与常规超声相比,超声造影可准确鉴别肾脏占位性病变的良恶性,诊断准确性、敏感性与特异性较高,便于指导临床治疗,改善患者预后。

Objective To investigate the diagnostic value of conventional ultrasound and contrast-enhanced ultrasonography for renal occupying lesions. Methods A total of 78 patients with renal occupying lesions admitted to our hospital from March 2016 to March 2019 were enrolled. All patients were examined by routine ultrasound and contrast-enhanced ultrasonography. The pathological diagnosis results were used as the gold standard, to analyze the value of conventional ultrasound and contrast-enhanced ultrasonography in the diagnosis of renal occupying lesions and the enhancement degree of contrast-enhanced contrast in different phases of major renal occupying lesions.Results The pathological diagnosis results of 78 patients showed 37 cases of malignant lesions and 41 cases of benign lesions;The sensitivity, specificity and accuracy of contrast-enhanced ultrasonography in the diagnosis of renal occupying lesions were 94.59%, 92.68%and 93.59%, respectively, significantly higher than 78.38%, 65.85%, and 71.79% of conventional ultrasound(P<0.05). The contrastenhanced intensity of renal clear cell carcinoma in medullary and cortical phase were significantly higher than those of smooth muscle lipoma(P<0.05), while no statistical difference was found in the contrast-enhanced intensity in regression phase between renal clear cell carcinoma and smooth muscle lipoma(P>0.05). Conclusions Compared with conventional ultrasound, contrast-enhanced ultrasonography can accurately identify benign and malignant renal occupying lesions, with higher diagnostic accuracy, sensitivity and specificity, which is convenient to guide clinical treatment and improve the prognosis of patients.
