管内分娩镇痛与产后腰背痛相关性的综述

A review of the correlation between intraspinal labor analgesia and postpartum low back pain

当下我国麻醉医学已进入推广分娩镇痛的实质性阶段,分娩后下背痛和腰痛与椎管内镇痛的相关性的问题引起了待产孕妇和产科麻醉医生的广泛关注。我国传统中医对黄种人女性的产后身痛有着优秀的研究成果,中医的产后身痛概念囊括了经产妇分娩后的下背痛和腰痛。国内认为椎管内麻醉下的剖宫产是经产妇产后腰背痛的相关因素。但尚未开展分娩后下背痛与椎管内分娩镇痛的相关性的研究。欧美国家在椎管内分娩镇痛普及后,在对经产妇的回顾性调查问卷研究中,经过统计学分析,发现分娩后的下背痛和腰痛与椎管内镇痛无相关性(尤其是慢性的产后疼痛)。我国麻醉领域目前尚缺乏类似欧美国家的回顾性研究(大样本)的实施和总结成果,因此有必要进行与我国女性相关的统计学研究。本文对孕妇分娩后的下背痛和腰痛与椎管内镇痛的相关性问题进行分析,并对我国女性和欧美白人的不同种族差异性的基础上进行综述。

At present,anesthesiologists in China have entered the substantive stage of promoting labor analgesia.The correlation between low back pain and osphyalgia after childbirth and intraspinal analgesia has attracted the attention of expectant pregnant women and obstetric anesthesiologists.Chinese traditional medicine has made excellent research results on postpartum bodily pain of women,and the concept of postpartum bodily pain in Chinese medicine includes lower back pain and osphyalgia after delivery.Cesarean section under intraspinal anesthesia was considered to be a related factor of postpartum low back pain in women.However,the correlation between postpartum low back pain and intraspinal labor analgesia has not been studied.After the popularity of intraspinal labor analgesia in European and American countries,in a retrospective questionnaire study on multipara women,after statistical analysis,it was found that there was no correlation between low back pain and osphyalgia after childbirth and intraspinal analgesia(especially chronic postpartum pain).There is still a lack of retrospective studies(large samples)similar to those in Europe and the United States in the field of anesthesia in China.It is necessary to carry out statistical studies related to women in China.To summarize the correlation between low back pain and osphyalgia after delivery and intraspinal analgesia on the basis of the racial differences between Chinese and in Europe and America women.
