前列腺增生症不同临床阶段术后三个月疗效的临床研究

Clinical Study on the Curative Effect of Prostatic Hyperplasia at Three Months after Operation at Different Clinical Stages

目的探讨前列腺增生(benign prostatic hyperplasia BPH)不同病理生理阶段手术疗效。方法收集2013年-2017年161例经尿道前列腺等离子剜除术术前尿动力学检测数据,根据尿动力学检查结果分为逼尿肌收缩力正常(A组)、逼尿肌收缩力正常伴有不稳定(B组)、逼尿肌收缩乏力(C组)。收集三组患者术前和术后3月的问卷调查前列腺增生症状评分(international prostate sym ptom scores,IPSS)、最大尿流率(mimum flow rate,Qmax)、残余尿(postvoid residual volume,PVR)、最大尿流率时逼尿肌压力(Pdet at Qmax)最大膀胱容量(Maximum cystometric capacity,MCC)的临床资料,分析及评价其临床疗效。结果三组患者术后3个月的IPSS、Qmax、PVR、Pdet at Qmax、MCC与术前进行组内比较,三组之间的组间比较,其中A组与C组差异很明显,但A组与B组明显弱化。结论经尿道前列腺等离子剜除术能够明显改善BPH患者排尿症状,及早手术治疗的患者术后效果确切,病情恢复快,能够减少患者的痛苦。

Objective To study the curative effect after surgery of different pathophysiological stage of operation for benign prostatic hyperplasia. Methods One hundred and sixty-one inpatients with BPH hospitalized in our department from June 2013 to 2017 were recruited in this study. They were divided into three groups according to the result of the urine dynamics test which the diuretic muscle contraction is normal, the muscle contraction force is normal and unstable, the diuretic muscle contractions are weak. International prostate symptom scores(IPSS), mimum flow-rate(Qmax),and postvoid residual volume(PVR) were performed for each patient both preoperatively and three months after surgery. To analyze and evaluate its clinical efficacy. Results There were statistically significant differences in mimum flow rate, postvoid residual volume, international prostate symptom scores and maximum cystometric capacity in each,groups,before,and after operatian(P<0.05). there were statistically significant differences in mimum flow rate, postvoid residual volume, international prostate symptom scores and, maximum cystometric capacity among the three groups after opratian(P<0.05), however, the difference between group A and group C was obvious,the difference between the intragroup was still statistically significant in the three groups at months(P<0.05), but the difference were significantly weakened in between the group A and the group B. Conclusions Transurethral Enucleative Resection of Prostate can significantly improve the symptoms of urination in patients with BPH, and the postoperative effect of early surgical treatment is accurate, and the recovery of the disease can reduce the patient′s pain.
