摘要
目的研究三叶因子3(TFF3)、环氧合酶-2(COX-2)、缺氧诱导因子-1α(HIF-1α)在不同胃黏膜病变中的表达意义及胃癌患者TFF3、COX-2、HIF-1α表达、微血管密度(MVD)的相关性比较。方法选取医院存储的行胃镜活检或胃癌根治术后蜡块标本150例,根据胃癌前不同病变期分为胃腺瘤组(B组,30例)、萎缩性胃炎伴肠化生组(C组,30例)、非典型增生组(D组,30例)、胃癌组(E组,30例),同期选取正常胃黏膜组(A组,30例)进行比较。结果无浆膜浸润、无淋巴转移、胃癌早期患者MVD值和TFF3、COX-2、HIF-1α表达阳性比率低于有浆膜浸润、有淋巴转移、胃癌进展期患者(P<0.05);高中分化癌患者MVD值和TFF3、COX-2、HIF-1α表达阳性比率与低分化癌患者无显著差异(P>0.05);TFF3、COX-2、HIF-1α表达阳性患者的MVD值显著高于TFF3、COX-2、HIF-1α表达阴性患者(P<0.05)。结论TFF3、COX-2、HIF-1α参与胃癌及癌前各期变化过程,TFF3、COX-2、HIF-1α、MVD对胃癌早期诊断具有重要意义,也是作为防治治疗胃癌的一个突破点,能否通过TFF3、COX-2、HIF-1α检测值对早期胃癌进行对应治疗,仍需临床医师深入研究。
Objective To investigate the expression of trefoil factor 3(TFF3),cyclooxygenase-2(COX-2)and hypoxia-inducible factor-1α(HIF-1α)in different gastric mucosal lesions and TFF3 and COX-2 in gastric cancer patients.Correlation between HIF-1αexpression and microvessel density(MVD).Methods 150 cases of wax specimens were collected from the hospital for gastroscopy biopsy or radical gastrectomy.According to different stages of gastric cancer,the gastric adenoma group(group B,30 cases),atrophic gastritis with intestinal metaplasia group(group C,30 cases),atypical hyperplasia group(group D,30 cases),gastric cancer group(Group E,30 cases),the normal gastric mucosa group(group A,30 cases)was selected for comparison.Results The MVD value and the positive rate of TFF3,COX-2 and HIF-1αexpression in patients without serosal invasion,lymphatic metastasis,early gastric cancer were lower than those with serosal invasion,lymphatic metastasis and advanced gastric cancer(P<0.05).The positive rate of MVD and TFF3,COX-2 and HIF-1αexpression in patients with high-grade differentiated cancer were not significantly different from those in poorly differentiated cancer(P>0.05).The MVD values of patients with positive expression of TFF3,COX-2 and HIF-1αwere significantly higher than those with negative expression of TFF3,COX-2 and HIF-1α(P<0.05).Conclusion TFF3,COX-2 and HIF-1αare involved in the process of gastric cancer and precancerous changes.TFF3,COX-2,HIF-1αand MVD are important for the early diagnosis of gastric cancer,and also a breakthrough point for the prevention and treatment of gastric cancer.Therefore,whether the TFF3,COX-2,and HIF-1αdetection values can be used for the corresponding treatment of early gastric cancer still requires in-depth study by clinicians.
