中药2号方联合健脾解毒汤治疗寻常型银屑病脾虚湿盛证的疗效观察及对外周血T淋巴细胞的表达影响

Therapeutic Effect of Traditional Chinese Medicine No.2 Prescription Combined with Jianpi Jiedu Decoction on Spleen Deficiency and Spleen Syndrome of Common Spleen and Expression of T Lymphocytes in Peripheral Blood

目的:中药2号联合健脾解毒汤治疗寻常型银屑病的临床疗效。方法:选取110例寻常型银屑病患者随机分为两组,对照组患者给予阿维A胶囊联合卡泊三醇软膏治疗,观察组患者采用中药2号联合健脾解毒汤治疗。比较治疗前后两组患者T淋巴细胞亚群、炎性因子水平、血液流变学、皮损改善情况、生活质量评价、瘙痒程度及临床疗效。结果:观察组的临床疗效显著优于对照组(P<0.05)。治疗后两组患者的CD+4、CD+4/CD+8及Treg均明显高于治疗前(P<0.05),Th17、Th17/Treg均明显低于治疗前(P<0.05),并且观察组患者各项指标显著高于对照组(P<0.05)。治疗后两组的TNF-α、IL-6、IL-17、IL-23及VEGF的水平均显著降低(P<0.05),并且观察组各项指标显著低于对照组,比较有统计学意义(P<0.05)。治疗后观察组患者的全血黏度、血浆黏度、红细胞沉降率及纤维蛋白原的降低幅度均较对照组显著(P<0.05)。治疗后两组的PASI、VAS及DLQI的评分均显著降低(P<0.05),并且观察组PASI、VAS及DLQI评分显著低于对照组,比较有统计学意义(P<0.05)。对照组不良反应发生率为29.1%,观察组不良反应发生率为7.2%,显著低于对照组(P<0.05)。结论:中药2号联合健脾解毒汤可以明显改善寻常型银屑病患者的临床症状,临床疗效确切,值得临床推广使用。

Objective:The clinical efficacy of Traditional Chinese Medicine No.2 Prescriptioncombined with Jianpi Jiedu Decoction in the treatment of psoriasis vulgaris.Methods:A total of 110 patients with psoriasis vulgaris were randomly divided into two groups.The patients in the control group were treated with Awei A Capsule combined with calcipotriol ointment.The patients in the observation group were treated with Traditonal Chinese Medicine No.2 Prescription combined with Jianpi Jiedu Decoction.The T lymphocyte subsets,inflammatory factor levels,hemorheology,skin lesion improvement,quality of life evaluation,degree of itching and clinical efficacy were compared between the two groups before and after treatment.Results:The clinical efficacy of the observation group was significantly better than that of the control group(P<0.05).After treatment,CD+4,CD+4/CD+8 and Treg were significantly higher in the two groups than those before treatment(P<0.05).Th17 and Th17/Treg were significantly lower than those before treatment(P<0.05),and the indicators in the observation group were significantly higher than those of the control group(P<0.05).After treatment,the levels of TNF-α,IL-6,IL-17,IL-23 and VEGF were significantly lower in the two groups(P<0.05)and the indicators in the observation group were significantly lower than those of the control group(P<0.05).After treatment,the whole blood viscosity,plasma viscosity,erythrocyte sedimentation rate and fibrinogen decrease were significantly higher in the observation group than those in the control group(P<0.05).The scores of PASI,VAS and DLQI in the two groups were significantly lower after treatment(P<0.05),and the scores of PASI,VAS and DLQI in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the control group was 29.1%,and that in the observation group was 7.2%,which was significantly lower than that in the control group(P<0.05).Conclusion:Traditional Chinese Medicine No.2 Prescription combined with Jianpi Jiedu Decoction can significantly improve the clinical symptoms of patients with psoriasis vulgaris.The clinical curative effect is definite and worthy of clinical promotion.
