黄芩汤联合二甲双胍治疗2型糖尿病疗效及对肠道菌群构成、胰岛素信号转导分子水平影响 被引量:25

摘要 目的:探讨黄芩汤辅助西药治疗糖尿病患者的疗效及对肠道菌群构成、胰岛素信号转导分子水平影响。方法:选取80例2型糖尿病门诊患者临床资料进行前瞻性研究,根据患者就诊的先后顺序,采用随机数字表达法将所有2型糖尿病患者分为治疗组与对照组,每组各40例。对照组采用基础西药治疗,治疗组在基础西药治疗基础上加黄芩汤治疗。比较两组患者的疗效、肠道菌群构成情况、胰岛素信号转导分子水平情况。结果:治疗组总有效率明显高于对照组(87.50%>60.00%),差异有统计学意义(P<0.05),结果提示,黄芩汤可以明显的改善糖尿病患者的临床症状。肠道菌群构成的结果提示,黄芩汤改变糖尿病患者的厚壁菌门(Firmicutes)和拟杆菌门(Bacteroidetes)两种细菌的构成。进一步对两组患者的胰岛素信号转导分子水平表达情况进行测定结果发现,黄芩汤对糖尿病患者胰岛素受体底物-1(IRS-1)、胰岛素受体底物-2(IRS-2)的上调和细胞因子信号转导抑制因子-3(SOCS-3)的下调均起到了加速的作用。结论:黄芩汤可能通过调控肠道菌群构成改善2型糖尿病,同时可改善胰岛素信号转导分子水平。 Objective: To investigate the effect of Huangqin Decoction on intestinal flora and insulin signal transduction molecular in diabetes. Method: This prospectivestudy had a total of 80 diabetes patients admitted in our hospital from June 2017 to December 2018 were analyzed. The patients were arranged in order according to their order of admission,and were divided into treatment group and control group by random number table method,40 cases in each group. The control group was treated with basic western medicine. The treatment group was treated with basic western medicine and Huangqin Decoction. After treatment, clinical efficacy, the levels of insulin signal transduction molecular and intestinal flora were compared in two groups. Result: The total effective rate in treatment group was higher than that in control group(87.50%>60.00%)(P<0.05). Results suggested that Huangqin Decoction can significantly improve the clinical symptoms in patients with diabetes. The results of intestinal flora suggested that Huangqin Decoction could change the compositions of Firmicutes and Bacteroidetes in diabetes patients.The further determination of insulin signal transduction molecular expression levels showed that Huangqin Decoction treatment promoted the up-regulation of IRS-1 and IRS-2 and down-regulation of SOCS-3 in diabetes patients.Conclusion:Huangqin Decoction improves the clinical efficacy of diabetes, which may be by regulating the intestinal flora of diabetes patients.Huangqin Decoction can significantly affect the expression of insulin signal transduction molecular in diabetes patients.
