肺超声检查在儿童肺炎的诊断及疗效评估中的价值

目的 探讨肺超声在儿童肺炎诊断与疗效评估中的应用价值.方法 将111例肺炎患儿行超声及胸部X线检查,超声检查的58例为观察组,X线检查的53例为对照组,比较二者的诊断价值及评估疗效.结果 肺超声与X线诊断结果比较,差异无统计学意义(P>0.05).肺炎患儿第一次超声征象主要体现为:A线消失100%;胸膜线异常100%;多重或融合的B线100%;肺实变33.3%.治疗5d后复查,A线消失50.9%;胸膜线异常63.2%;多重或融合的B线71.9%;肺实变52.6%.出院时复查结果显示上述异常征象基本消失.结论 肺超声检查对儿童肺炎诊断的准确性与X线相当,在疗效判断中的应用效果显著,能够提供准确的病情变化数据,可减少儿童在X线下的暴露.

Objective To investigate the application value of lung ultrasound in diagnosis and curative effect evaluation of pneumonia in children.Methods 111 children with pneumonia were divided into two groups.58 case of X-ray and ultrasound examination,ultrasound review were used as research group.53 case of X-ray examination and reexamination were used as control group.The diagnostic value of two groups were compared and the therapeutic effect were evaluated.Results Comparison of lung ultrasound and X-ray diagnosis,the difference was not statistically significant(P>0.05).The first ultrasonic signs of children with pneumonia were mainly manifested as:A-line off(100%),abnormal pleural line(100%),B-line increase(100%),pulmonary consolidation(33.3%).After treated for 5 days,50.9%prtients still had one or more areas with A-line disappear,63.2%patients still with pleural line abnormal,71.9%patients had one or more areas with B-line increase,and 52.6%patients with pulmonary consolidation.The results of the child’s discharge showed that the above abnormal signs disappeared.Conclusion The accuracy of lung ultrasound in the diagnosis of pneumonia in children is no less than X-ray.The effect is remarkable in the judgement of curative effect,able to provide accurate date on disease changes,it can reduce children's exposure to X-rays.
