右美托咪定复合羟考酮用于纤维支气管镜检查术的临床观察 被引量:3

摘要 目的 探讨右美托咪定复合羟考酮用于纤维支气管镜检查术的麻醉效果.方法 择期行纤维支气管镜检查术患者60例,随机分为观察组(A组)与对照组(B组),每组各30例.A组静脉注射羟考酮0.1mg/kg,右美托咪定1μg/kg静脉泵注进行诱导,术中右美托咪定0.2μg/(kg·min)泵注维持;B组静脉注射芬太尼1μg/kg、丙泊酚1~2mg/kg进行诱导,术中丙泊酚3mg/(kg·h)泵注维持.观察不同时点HR、MAP、SpO2的变化.对患者的麻醉效果进行评价,记录患者呛咳、体动、憋气、气道痉挛等不良反应的发生情况.结果 纤维支气管镜检查期间A组较B组心率控制良好、血压波动小,SpO2明显高于B组(P<0.05).A组麻醉效果优良率明显高于B组(P<0.05),呛咳、体动的发生率明显低于B组(P<0.05).结论 右美托咪定复合羟考酮用于纤维支气管镜检查术,麻醉效果安全有效,血流动力学稳定,不良反应小. Objective To observe the efficacy of dexmedetomidine combined with oxycodone during fiberoptic bronchoscope examination.Methods Sixty patients underwent fiberoptic bronchoscope examination were randomly divided into group A and B,30 in each group.Group A received intravenous infusion of oxycodone 0.1mg/kg and a loading dose of dexmedetomidine 1μg/kg as induction of anesthesia,followed by a continuous infusion at a rate of 0.2μg/(kg·min)until the end of surgery.An initial induction dose of fentanyl 1μg/kg and propofol 1~2mg/kg were given followed by an maintaining infusion of propofol 3 mg/(kg·h)in group B.HR,MAP and SpO2 values of different time points were recorded.Adverse reactions and anesthetic effect were recorded.Results There were less intraoperative hemodynamic fluctuations in Group A compared with Group B.Intraoperative SpO2 in Group A were higher than the corresponding values in Group B(P<0.05).The superior rate of anesthetic effect was better in Group A than Group B(P<0.05).The incidences of bucking and body movement in group A were significantly lower than those in group B(P<0.05).Conclusion Dexmedetomidine combined with oxycodone during fiberoptic bronchoscope examination shows satisfactory anesthesia effects.
