特发性膜性肾病发病机制研究进展 被引量:3

摘要 特发性膜性肾病(IMN)是一种自身免疫性足细胞病,典型的病理表现是肾小球基底膜弥漫性增厚和上皮下免疫复合物的沉积.IMN是中老年人原发性肾病综合征(NS)的常见疾病,国外报道占成人NS的20%,在中老年NS患者中可达60%[1].随着诊断水平的提高,在原发性肾小球疾病中IMN所占比例有着显著的上升趋势.朱平等[2]研究发现在原发性肾小球疾病中IMN比例由2003~2007年的16.80%上升至2008~2012年的29.35%.IMN的自然病程差异较大,约1/3的患者存在自发缓解情况,而另1/3的患者在发病后5~10年内可进展为慢性肾衰竭[3].研究证实磷酯酶A2受体(PLA2R)是IMN发病的重要靶抗原,其特异性100%,敏感性80%[4].
