术中超声在胰腺体尾部切除术中的辅助应用价值

Value of intraoperative ultrasound in auxiliary application of distal pancreatectomy

目的探讨术中超声在胰腺体尾部切除术中的辅助应用价值。方法回顾性分析71例胰腺体尾部肿瘤性病变并行胰腺体尾部切除术患者的临床资料与术中超声检查结果。以术后切除的肿瘤组织病理检查结果为金标准,与术前CT检查及术中超声检查作比较。结果术中超声检查用时(18±5)min,肿瘤检出率为100%。根据术后病理检查结果,术中超声检查与术前CT检查的诊断准确率分别为90.1%和95.8%(P=0.453)。2例患者术前CT检查未提示血管侵犯而术中超声提示血管侵犯;6例因术中超声检查而改变手术方式。结论术中超声对病变的定位准确,能检测血管侵犯及能显示病变周围组织关系,对手术有指导作用。

Objective To explore the value of intraoperative ultrasound in the treatment of distal pancreatectomy.Methods Retrospective analysis was performed on the clinical data and intraoperative ultrasound results of 71 patients with pancreatic body-tail tumors.The results of postoperative histopathological examination of the tumor tissues were used as the gold standard,and were compared with the results of preoperative CT examination and intraoperative ultrasound examination.Results The time of intraoperative ultrasound was(18±5)min,and the tumor detection rate was 100%.According to the results of postoperative pathological examination,the diagnostic rates of intraoperative ultrasound examination and preoperative CT examination were 90.1%and 95.8%,respectively(P=0.453),and the results showed no statistical difference.Intraoperative ultrasound indicated vascular invasion in 2 patients but preoperative CT examination did not.The operation mode of 6 patients was changed by intraoperative ultrasound examination.Conclusions Intraoperative ultrasound can locate the lesion accurately,detect vascular invasion,and show the relationship of surrounding tissues,which has a guiding significance for surgery.
