中枢神经细胞瘤的MR表现及其鉴别诊断

MR Manifestations and Differential Diagnosis of Central Neurocytoma

目的 分析中枢神经细胞瘤的MR表现,以提高对该病诊断准确率.方法 对13 例经手术病理证实的中枢神经细胞瘤的MR表现进行回顾性分析,总结其影像特征.结果 13 例患者中,12 例位于侧脑室内,10 例邻近Monro孔、与透明隔宽基底相连;病灶多为囊实性,11 例可见"皂泡征",8 例矢状位可见"扇形征";增强扫描11 例呈轻-中度强化,2 例呈不均匀明显强化,9 例病灶内部及周边见强化血管影;8 例伴有不同程度侧脑室积水.结论 中枢神经细胞瘤临床、影像及病理学表现有一定特征性,结合发病年龄及典型特点,有助于与其它脑室内肿瘤鉴别.

Objective To improve the diagnostic accuracy of central neurocytoma by analyzing the MR findings of this tumor. Methods The MR findings of 13 cases of central neurocytoma confirmed by operation and pathology were retrospectively analyzed and their imaging features were summarized. Results Of the 13 patients, 12 were located in the lateral ventricle, 10 were adjacent to the Monro hole and connected to the transparent septum. The lesions were mostly cystic, with 11 cases showing "soap bubble sign", and 8 cases showing "sector sign" in sagittal position. 11 cases showed mild-to-moderate enhancement, 2 cases showed inhomogeneous obvious enhancement, and 9 cases showed enhanced vascular in and around the lesion. 8 cases had different degrees of hydrocephalus. Conclusion The clinical, imaging and pathological manifestations of central neurocytoma show some characteristics. The typical characteristics, when combined with the age of onset, are helpful to distinguish this disease from other intraventricular tumors.
