大黄牡丹汤加味联合西药治疗阑尾周围脓肿临床研究

Clinical Study on Modified Dahuang Mudan Tang Combined with Western Medicine for Periappendicular Abscess

目的:观察大黄牡丹汤加味联合西医方案治疗阑尾周围脓肿的临床疗效。方法:将146例阑尾周围脓肿患者按随机数字表法分为观察组和对照组各73例。对照组采用常规西医抗生素治疗,观察组在对照组的基础上加用大黄牡丹汤随症加减治疗。比较2组的治疗效果和恢复情况。结果:治疗后,观察组发热缓解时间、腹痛缓解时间、肿块消散时间及平均治愈时间均低于对照组,差异均有统计学意义(P<0.05)。治疗前,2组白细胞计数、中性粒细胞百分比、C-反应蛋白水平比较,差异无统计学意义(P>0.05)。治疗后,2组白细胞计数、中性粒细胞百分比、C-反应蛋白水平较治疗前降低(P<0.05),且观察组白细胞计数、中性粒细胞百分比、C-反应蛋白水平均低于对照组(P<0.05)。观察组总有效率为95.9%,显著高于对照组75.3%,差异有统计学意义(P<0.05)。结论:大黄牡丹汤加味联合西医治疗阑尾周围脓肿疗效肯定,值得推广。

Objective:To observe the clinical effect of modified Dahuang Mudan tang combined with western medicine for periappendicular abscess.Methods:A total of 146 cases of patients with periappendicular abscess were divided into the observation group and the control group according to the random number table method,73 cases in each group.The control group was treated with routine western medicine--antibiotics,and the observation group was additionally treated with modified Dahuang Mudan tang based on the treatment of the control group.The curative effect and the recovery in the two groups were compared.Results:After treatment,the remission time of fever and abdominal pain,the dissipating time of masses and the average cure time in the observation group were shorter than those in the control group,the difference being significant(P<0.05).Before treatment,compared the white blood cell count,the percentages of neutrophils and the levels of C-reactive protein in the two groups,there was no significance in the difference(P>0.05).After treatment,the white blood cell count,the percentages of neutrophils and the levels of C-reactive protein in the two groups were decreased when compared with those before treatment(P<0.05),and the ones in the observation group were lower than those in the control group(P<0.05).The total effective rate was 95.9%in the observation group,significantly higher than that of 75.3%in the control group,the difference being significant(P<0.05).Conclusion:The therapy of modified Dahuang Mudan tang combined with western medicine in treating periappendicular abscess has defined curative effect,which is worthy of promotion.
