中医综合疗法联合解剖钢板治疗膝关节周围骨折临床研究

Clinical Study on Chinese Medicine Comprehensive Therapy Combined with Anatomical Plate for Fractures Around Knee Joint

目的:探讨中医综合疗法联合解剖钢板治疗膝关节周围骨折的疗效。方法:将膝关节周围骨折患者68例,随机分为观察组和对照组,每组均34例。对照组采用单一解剖钢板固定治疗,观察组在对照组治疗的基础上联合中药内服、外敷治疗及穴位按摩的综合疗法。比较2组治疗前后的疼痛、肿胀、瘀斑、神疲乏力症状评分,2组伤口愈合情况、以及疼痛消失时间、肿胀消失时间、切口愈合时间、骨病出现时间。结果:2组治疗前疼痛、肿胀、瘀斑及神疲乏力评分比较,差异无统计学意义(P>0.05)。治疗后,2组各项症状评分均较术前降低(P<0.05)。观察组治疗后的疼痛、肿胀、瘀斑及神疲乏力评分低于对照组(P<0.05)。2组治疗前膝关节活动度比较,差异无统计学意义(P>0.05)。治疗12周、24周后,2组的膝关节活动度均较治疗前增大(P<0.05)。观察组治疗12周、24周后的膝关节活动度高于对照组(P<0.05)。观察组膝关节活动优良率高于对照组(P<0.05)。观察组住院时间为(12.44±3.01)天,短于对照组(16.20±3.73)天,差异有统计学意义(t=4.574,P<0.000)。结论:中医综合疗法联合解剖钢板治疗膝关节周围骨折能有效改善患者膝关节活动度,缓解和消退患者疼痛、肿胀等症状,可减轻患者痛苦,住院时间更短,康复效果更理想。

Objective:To explore the clinical effect of Chinese medicine comprehensive therapy combined with anatomical plate for fractures around knee joint.Methods:Divided 68 cases of patients with fractures around knee joint into the observation group and the control group randomly,34 cases in each group.The control group received fixation with anatomical plate simply,while the observation group additionally received Chinese medicine via oral administration and external application and acupoint massage for comprehensive treatment.Before and after treatment,compared the symptom scores of pain,distention,ecchymosis and lassitude in both groups,the status of wound healing in both groups,the disappearance time of pain,the disappearance time of distention,the time for wound healing and the appearance time of callus in both groups.Results:Before treatment,comparing the scores of pain,distention,ecchymosis and lassitude in both groups,there was no significant difference being found(P>0.05).After treatment,the scores of pain,distention,ecchymosis and lassitude in both groups were lower than those before treatment(P<0.05);each symptom score in the observation group was lower than that in the control group(P<0.05).Before treatment,comparing the range of motion of knee joint in both groups,there was no significant difference being found(P>0.05).After treatment for 12 and 24 weeks,the range of motion of knee joint in both group was larger than that before treatment(P<0.05);the range of motion of knee joint in the observation group was larger than that in the control group(P<0.05).The excellence rate of knee joint motion in the observation group was higher than that in the control group(P<0.05).The duration of hospitalization was(12.44±3.01)days in the observation group,being shorter than(16.20±3.73)days in the control group,difference being significant(t=4.574,P<0.000).Conclusion:Chinese medicine comprehensive therapy combined with anatomical plate for fractures around knee joint can effectively improve the range of motion of knee joint,relieve and dispel such symptoms as pain and distention and shorten the duration of hospitalization,of which the rehabilitation effect is optimal.
