温针灸联合身痛逐瘀汤治疗腰椎间盘突出症临床研究

Clinical Study on Warm Needling Combined with Shentong Zhuyu Tang for Lumbar Disc Herniation

目的:探析温针灸联合身痛逐瘀汤对腰椎间盘突出症患者日本骨科协会评估治疗(Japanese Orthopaedic Association,JOA)评分、腰背肌力及生活自理能力的影响。方法:依据随机数字表法将2016年12月-2018年12月期间甘肃省中医院收治的92例腰椎间盘突出症患者分为2组,对照组46例接受单纯身痛逐瘀汤治疗,研究组46例接受温针灸联合身痛逐瘀汤治疗,比较2组临床疗效、JOA评分、腰背肌力与腹肌肌力变化情况、生活自理能力、脊柱前凸角与脊柱后凸角变化情况。结果:研究组总有效率明显高于对照组(P<0.05);治疗后2组JOA评分较治疗前明显增加(P<0.05),且研究组明显高于对照组(P<0.05);治疗后2组腰背肌力、腹肌肌力及脊柱前凸角较治疗前明显增强(P<0.05),脊柱后凸角较治疗前明显降低(P<0.05),且研究组明显优于对照组(P<0.05);治疗后2组生活自理能力评分较治疗前明显增加(P<0.05),且研究组明显高于对照组(P<0.05)。结论:身痛逐瘀汤与温针灸治疗腰椎间盘突出症,相得益彰,疗效理想,有利于增加JOA评分、增强腰背肌力与腹肌肌力、提高患者生活自理能力、改善脊柱前凸角与脊柱后凸角,临床疗效较好。

Objective:To discuss and analyze the effect of warm needling combined with Shentong Zhuyu tang for lumbar disc herniation on the Japanese Orthopaedic Association(JO A)score,the muscle strength of low back and the self-care ability.Methods:A total of 92 cases of patients with lumbar disc herniation treated in Gansu Provincial Hospital of Traditional Chinese Medicine from Dec.2016 to Dec.2018 were divided into two groups according to the random number table method,46 cases in each group.The control group was treated with simple Shentong Zhuyu tang,and the study group was additionally treated with warm needling based on the treatment of the control group.The clinical effect,the changes of JO A scores,muscle strengths of low back and abdominal muscle,self-care ability,and lordosis angle as well as kyphosis angle in the spine in the two groups were compared.Results:The total effective rate in the study group was significantly higher than that in the control group(P<0.05).After treatment,the JOA scores in the two groups were significantly increased when compared with those before treatment(P<0.05),and the score in the study group was significantly higher than that in the control group(P<0.05).After treatment,the muscle strengths of low back as well as abdominal muscle and lordosis angles in the two groups were significantly increased when compared with those before treatment(P<0.05),and the kyphosis angles were significantly decreased(P<0.05),and the above indexes in the study group were significantly better than those in the control group(P<0.05).After treatment,the scores of self-care ability in the two groups were significantly increased when compared with those before treatment(P<0.05),and the score in the study group was significantly higher than that in the control group(P<0.05).Conclusion:The therapy of Shentong Zhuyu tang combined with warm needling can achieve ideal curative effect in treating lumbar disc herniation by complementing each other,which is beneficial to increasing the JOA scores and muscle strengths of low back as well as abdominal muscle,improving self-care ability and lordosis angle as well as kyphosis angle in the spine,and with good clinical effect.
