温针疗法联合玻璃酸钠关节腔注射治疗膝关节骨性关节炎临床研究

Clinical Study on Warm Needling Combined with Intra-articular Injection of Sodium Hyaluronate for Knee Osteoarthritis

目的:观察温针疗法联合玻璃酸钠关节腔注射治疗膝关节骨性关节炎的临床疗效及对机体炎症因子的影响。方法:选取90例膝关节骨性关节炎患者为研究对象,按随机数字表法分为观察组和对照组,每组45例。对照组给予玻璃酸钠注射液关节腔注射治疗,观察组在此基础上增加温针疗法治疗。对比2组临床疗效、疼痛程度、膝关节功能和炎症因子水平。结果:观察组总有效率88.89%,高于对照组的71.11%,差异有统计学意义(P<0.05)。治疗后,2组视觉模拟评分法(VAS)评分均较治疗前降低(P<0.05),Lysholm膝关节评分均较治疗前升高(P<0.05);观察组VAS评分低于对照组(P<0.05),Lysholm膝关节评分高于对照组(P<0.05)。治疗后,2组C-反应蛋白(CRP)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α(TNF-α)水平均较治疗前降低,观察组以上3项炎症因子水平均低于对照组,差异均有统计学意义(P<0.05)。结论:温针疗法联合玻璃酸钠关节腔注射治疗膝关节骨性关节炎临床疗效显著,可以减轻患者的疼痛程度,改善膝关节功能,降低炎症因子水平。

Objective:To observe the clinical effect of the therapy of warm needling combined with intra-articular injection of sodium hyaluronate for knee osteoarthritis and its effect on inflammatory cytokines in the body.Methods:A total of 90 patients with knee osteoarthritis were selected as the study subjects and divided into the observation group and the control group according to the random number table method,45 patients in each group.The control group was treated with intra-articular injection of sodium hyaluronate and the observation group was additionally treated with warm needling.The clinical effect,pain degree,knee function and the level of inflammatory cytokines were compared between the two groups.Results:The total effective rate was 88.89% in the observation group,higher than that of 71.11% in the control group,the difference being significant(P <0.05).After treatment,the VAS scores in both groups were decreased when compared with those before treatment(P <0.05),and Lysholm knee scores were increased when compared with those before treatment(P <0.05).The VAS score in the observation group was lower than that in the control group(P <0.05),while Lysholm knee score was higher than that in the control group(P <0.05).After treatment,the levels of C-reactive protein(CRP),interleukin-6(IL-6) and tumor necrosis factor-a(TNF-a) in the two groups were decreased when compared with those before treatment,and the levels of the above three inflammatory cytokines in the observation group were all lower than those in the control group,differences being significant(P <0.05).Conclusion:The therapy of warm needling combined with intra-articular injection of sodium hyaluronate has significant clinical effect in treating knee osteoarthritis,which can reduce the degree of pain,improve knee function and decrease the level of inflammatory cytokines.
