百会穴针刺加穴位注射治疗脑梗死后抑郁症临床观察

Feasibility study of Baihui acupuncture plus acupoint injection for treatment of post-cerebral infarction depression

[目的]探讨百会穴针刺加穴位注射治疗脑梗死后抑郁症的可行性研究。[方法]将2018年2月-2019年2月在医院神经内科治疗的78例脑梗死后抑郁症患者随机分为两组,均给予常规脑梗死治疗,在此基础上,对照组使用盐酸舍曲林片治疗,观察组使用百会穴针刺加穴位注射治疗,比较两组患者的临床疗效、神经功能及抑郁各量表评分变化、神经递质水平变化、炎性因子水平变化。[结果]观察组治疗有效率为94.87%,明显高于对照组的74.36%(P<0.05);观察组治疗后SDS、HAMD、NIHSS评分明显低于对照组,而MMSE、BI指数明显高于对照组(P<0.05);观察组治疗后TNF-α、hs-CRP、IL-6、IL-8水平明显低于对照组(P<0.05);两组治疗后5-HT、ET水平明显高于治疗前(P<0.05),但两组治疗后5-HT、ET水平无明显差异(P>0.05)。[结论]百会穴针刺加穴位注射治疗脑梗死后抑郁症的效果显著,提高脑部神经低脂水平,有效降低抑郁程度,改善神经功能缺损,减轻体内炎症水平,具有积极的临床意义。

[Objective]To investigate the feasibility of acupuncture plus acupoint injection at Baihui Point for the treatment of post-cerebral infarction depression.[Methods]78 patients with post-cerebral infarction treated with neurology in our hospital from February 20 to February 2019 were randomly divided into two groups.All patients were treated with conventional cerebral infarction.On this basis,the control group used hydrochloric acid.Treatment with Qulin tablets,observation group using Baihui acupuncture plus acupoint injection therapy,comparing the clinical efficacy,neurological function and depression scores,neurotransmitter levels,inflammatory factors levels in the two groups.[Results]The effective rate of the observation group was 94.87%,which was significantly higher than that of the control group(74.36%)(P<0.05).The SDS,HAMD and NIHSS scores in the observation group were significantly lower than those in the control group,while the MMSE and BI indexes were significantly higher than the control group.The levels of TNF-α,hs-CRP,IL-6 and IL-8 in the observation group were significantly lower than those in the control group(P<0.05).The levels of 5-HT and ET were significantly higher after treatment.Before treatment(P<0.05),there was no significant difference in 5-HT and ET levels between the two groups(P>0.05).[Conclusion]Baihui acupuncture plus acupoint injection is effective in treating depression after cerebral infarction,improving brain low blood lipid level,effectively reducing depression,improving neurological deficit and reducing inflammation level in the body.It has positive clinical significance.
