摘要
目的通过对228例流产孕妇绒毛组织进行细胞染色体核型分析,研究绒毛染色体异常状况,以及胎儿染色体异常与孕妇年龄及生育史之间的关系。方法对2017年1月至2018年12月因自然流产在本院进行绒毛组织培养和染色体核型分析的228例患者进行回顾性分析。结果228例绒毛组织中,培养成功218例,培养失败10例,培养成功率95.6%。218例流产绒毛中共检测出异常核型样本105例,占比为48.2%,正常核型样本113例,占比为51.8%。在异常核型中,X染色体单体及三体异常占比较多,分别为11.5%及27.5%。35岁以下患者和35岁以上患者(包括35岁)流产绒毛核型异常样本率比较差异具有统计学意义(P<0.05),有正常生育史的患者与没有生育过正常小孩的患者流产绒毛异常核型样本数目比较,差异无统计学意义。结论胎儿染色体异常是导致孕妇自然流产的主要原因之一,对已发生自然流产的孕妇提供绒毛染色体核型分析可以辅助临床分析患者流产原因,明确诊断,科学合理指导患者下一次妊娠。
Objective To study the chromosomal abnormalities of the chorionic villi in 228 cases of spontaneous abortion.Exploring the relation-ship between the chromosomal abnormalities and the gestation age,reproductive history.Methods 228 abortion villis,which were conducted cell culture and karyotyping,were analyzed retrospectively between January 2017 and December 2018.Results In 228 cases of abortion villi,218 cases were cultured successfully,10 cases failed(success rate 95.6%).In 218 cultured successful cases,105 cases were detected chromosomal abnormalities(accounting for 48.2%),113 cases were normal(accounting for 51.8%).In the cases of chromosomal abnormalities,X monosomy and autosomal trisomies were accounting for 11.5%and 27.5%respectively.The abnormal chromosomal rates between the patients under 35 years old and over 35 years old(including 35 years old)were significantly difference(P<0.05).There was no significant difference for the abnormal chromosomal rates between the patients with normal fertility history and others with abnormal fertility history.Conclusion Chromosomal abnormality are the primary cause of spontaneous abortion.The techlogy of karyotype analysis is conducive to analyze chorionic villi tissues for the patients with spontaneous abortion,which will assist clinical analysis.and diagnosis,then give scientific and reasonable guidance for the next pregnancy.
