肠内外联合营养支持治疗对老年食管癌患者的影响

Impact of enteral and parenteral nutrition support in elderly esophagus cancer patients

目的研究肠内外联合营养支持治疗对老年食管癌患者的影响。方法选取2018年6月至2019年6月本院收治的84例老年食管癌患者为研究对象,根据病例单双号分为单一组42例(接受肠外营养支持治疗)和联合组42例(接受肠外及肠内联合营养支持治疗),统计并比较两组患者治疗前后的营养状况、免疫功能情况以及生活质量。结果治疗前,两组血清总蛋白、清蛋白、前清蛋白以及转铁蛋白水平比较差异均无统计学意义;治疗后,两组营养状况各指标水平均有所改善,但联合组改善程度优于单一组(P<0.05);治疗前,两组免疫球蛋白G、免疫球蛋白A、免疫球蛋白M以及血总淋巴细胞计数水平比较差异均无统计学意义;治疗后,两组免疫功能各指标水平均有所改善,但联合组改善程度优于单一组(P<0.05);治疗后联合组躯体功能、认知功能、社会功能、情绪功能以及角色功能5个维度生活质量评分均高于单一组(P<0.05)。结论在对老年食管癌患者实施肠外营养支持治疗的同时再给予肠内营养支持治疗,利于患者营养状况、免疫功能的改善,并提高患者生活质量。

Objective To study the impact of enteral and parenteral nutrition support in elderly esophagus cancer patients.Methods 84 elderly esophagus cancer patients in our hospital were selected from June 2018 to June 2019 and assigned to single group(n=42)and combined group(n=42),according to odd or even medical record sheet number.Patients were treated with parenteral nutrition support alone or enteral and parenteral nutrition support,respectively.The nutritional status,immune function and life quality were probed.Results Before treatment,the values of total protein,albumin,prealbumin and transferrin between groups were not significantly different.After treatment,the nutritional index for two groups were improved,and related index in the combined group were better than single group(P<0.05).Before treatment,the values of immunoglobulin G(IgG),immunoglobulin A(IgA),immunoglobulin M(IgM)and total lymphocyte counts between groups were not significantly different.After treatment,the immune function index for two groups were improved,and related index in the combined group were better than single group(P<0.05).The scores of physical function,cognitive function,social function,emotional function and role function in the combined group were higher than single group(P<0.05).Conclusion The enteral and parenteral nutrition support can improve the patient’s nutritional status,immune function and life quality.
