防旋股骨近端髓内钉微创固定治疗股骨粗隆间骨折的疗效分析

Therapeutic Effect of Minimally Invasive Fixation of Proximal Femoral Nail in the Treatment of Femoral Intertrochanteric Fracture

目的研究防旋股骨近端髓内钉微创固定术在股骨粗隆间骨折中的应用价值。方法随机选取2016年3月—2018年2月该院接诊的股骨粗隆间骨折病患60例,并将之采用奇偶数字分组法分成试验和对照两组(n=30)。试验组接受防旋股骨近端髓内钉微创固定治疗,而对照组则接受动力髋螺钉固定治疗。分析两组术后髋关节功能的恢复情况,并对各组的手术用时等指标作出比较。结果试验组的术中出血量为(173.58±65.19)mL,比对照组的(298.04±45.23)mL少(t=8.592,P<0.05)。试验组的手术用时和术后伤口愈合时间比对照组短(P<0.05)。试验组的术后并发症发生率为3.33%,比对照组的20.00%低(χ2=4.043,P<0.05)。试验组术后10个月的髋关节功能评分为(89.22±7.93)分,比对照组的(77.24±7.06)分高(t=6.180,P<0.05)。结论于股骨粗隆间骨折临床治疗中运用防旋股骨近端髓内钉微创固定疗法,术后并发症少,手术用时短,伤口愈合快,髋关节功能改善明显,建议临床推广。

Objective To study the value of minimally invasive fixation of the proximal femoral nail in the treatment of femoral intertrochanteric fractures.Methods Sixty patients with femoral intertrochanteric fractures who were admitted to the hospital from March 2016 to February 2018 were randomly selected and divided into two groups(n=30)by parity and digital grouping.Among them,the experimental group received minimally invasive fixation of the proximal femoral nail of the anti-rotation femur,while the control group received dynamic hip screw fixation.The recovery of hip joint function after operation was analyzed,and the indexes of operation time and time of each group were compared.Results The intraoperative blood loss of the experimental group was(173.58±65.19)mL,which was less than that of the control group(298.04±45.23)mL(t=8.592,P<0.05).The time of surgery and postoperative wound healing in the experimental group were shorter than those in the control group(P<0.05).The postoperative complication rate of the experimental group was 3.33%,which was lower than that of the control group,20.00%(χ2=4.043,P<0.05).The hip function score of the experimental group was(89.22±7.93)points at 10 months after operation,which was higher than that of the control group(77.24±7.06)points(t=6.180,P<0.05).Conclusion In the clinical treatment of intertrochanteric fractures,the use of anti-rotation proximal femoral nail minimally invasive fixation therapy,less postoperative complications,short operation time,rapid wound healing,and improved hip function,which is worthy promotion in clinic.
