氧氟沙星加氟米龙局部点眼联合泪道加压冲洗治疗泪道狭窄的临床研究

Clinical Study of Ofloxacin Plus Fluorometholone Topical Eye Combined with Lacrimal Duct Flushing in the Treatment of Lacrimal Stenosis

目的对比单纯使用氧氟沙星+氟米龙局部点眼和氧氟沙星+氟米龙局部点眼联合泪道加压冲洗治疗泪道狭窄的有效性、安全性、复发率。方法方便选择2016年3月—2019年6月该院60例泪道狭窄患者。随机分为治疗组A(单纯药物局部点眼)20例;治疗组B(药物局部点眼联合泪道加压冲洗qw)20例;治疗组C(药物局部点眼联合泪道加压冲洗biw)20例。比较研究对象总体有效率、复发率、不良反应发生率情况。结果治疗1个月后,3个治疗组的总体有效率分别为40.00%、75.00%和80.00%。治疗组B和C相对于治疗组A,总体有效率差异有统计学意义(P<0.05)。随访4个月后,3个治疗组的复发率分别为75.00%、26.67%和25.00%。治疗组B和C相对于治疗组A,复发率差异有统计学意义(P<0.05)。所有患者治疗期间,眼压均无显著性变化,鼻腔无疼痛、出血,裂隙灯下观察泪小点无充血、红肿。结论氧氟沙星qid+氟米龙tid局部点眼联合泪道加压冲洗可显著提高泪道狭窄的治疗有效性,同时有效减少复发率。该方法安全、有效,建议临床推广。

Objective To compare the efficacy,safety and recurrence rate of simple use of ofloxacin+fluorometholone local eye and ofloxacin+fluorometholone local eye and lacrimal pressure irrigation in the treatment of lacrimal stenosis.Methods 60 patients with lacrimal stenosis were convenient enrolled in our hospital from March 2016 to June 2019.Randomly divided into treatment group A(single drug local eye)20 cases;treatment group B(drug local eye combined with lacrimal pressure irrigation qw)20 cases;treatment group C(drug local eye combined with lacrimal pressure washing biw)20 cases.The overall efficiency,recurrence rate,and incidence of adverse reactions were compared between the subjects.Results After one month of treatment,the overall effective rates of the three treatment groups were 40.00%,75.00%,and 80.00%,respectively.There was a statistically significant difference in overall efficiency between treatment groups B and C versus treatment group A(P<0.05).After 4 months of follow-up,the recurrence rates of the three treatment groups were 75.00%,26.67%,and 25.00%,respectively.There was a statistically significant difference in the recurrence rate between the treatment groups B and C relative to the treatment group A(P<0.05).During the treatment of all patients,there was no significant change in intraocular pressure,no pain or bleeding in the nasal cavity,and no puncture or redness was observed under the slit lamp.ConclusionLevloxacinqid+flumilontid combined with dacryocystorhinoplasty can significantly improve the treatment effectiveness of dacryocystorhinostenosis and reduce the recurrence rate.The method is safe and effective.
