手术治疗老年髋部骨折术后对侧髋骨再骨折的研究

Operative Treatment of Contralateral Hip Re-fracture in Elderly Patients with Hip Fracture

目的探讨老年髋部骨折患者的手术治疗效果,观察其术后对侧髋骨再骨折的发生情况。方法便利选取该院2018年3月—2019年1月期间收治的94例老年髋部骨折患者作为研究对象,分为观察组(手术治疗)和对照组(保守治疗),各47例,观察两组患者的术后恢复情况。结果与对照组相比,观察组患者的治疗有效率(97.87%>80.85%,χ2=7.162)、满意度(93.62%>76.60%,χ2=5.371)、健康调查简表(SF-36)评分[(68.51±3.94)分>(61.22±3.18)分,t=5.715]以及Harris髋关节评分[(65.42±6.55)分>(51.19±4.83)分,t=5.193]相对更高(P<0.05),而术后对侧髋骨再骨折发生率(6.38%<27.66%,χ2=7.532)、焦虑自评量表(SAS)评分[(39.22±4.18)分<(48.34±5.06)分,t=5.264]相对更低(P<0.05),住院时间[(16.17±4.35)d<(26.34±3.95)d,t=5.658]相对更短(P<0.05)。结论在老年髋部骨折的临床治疗中,手术治疗是一种安全、有效的治疗方法,在修复骨伤的同时,术后对侧髋骨再骨折的发生率低,能够有效减少术后康复期间的干扰因素。

Objective To investigate the surgical treatment effect of elderly patients with hip fracture and to observe the occurrence of contralateral hip re-fracture after operation.Methods 94 elderly patients with hip fracture treated in the hospital from March 2018 to January 2019 were convenienty selected and divided into observation group(surgical treatment)and control group(conservative treatment),47 cases in each group,and the postoperative recovery of the two groups was observed.Results Compared with the control group,the effective rate of treatment(97.87>80.85%,χ2=7.162),satisfaction(93.62%>76.60%,χ2=5.371),health questionnaire(SF-36)score[(68.51±3.94)points>(61.22%±3.18)points,t=5.715]and Harris hip score[(65.42±6.55)points>(51.19±4.83)points,t=5.193]in the observation group were relatively higher(P<0.05),while the incidence of contralateral hip re-fracture(6.38%<27.66%,χ2=7.532)was higher(P<0.05).The self-rating anxiety scale(SAS)score[(39.22±4.18)points<(48.34±5.06)points,t=5.264]was lower(P<0.05),and the length of hospitalization[(16.17±4.35)d<(26.34±3.95)d,t=5.658]was shorter(P<0.05).Conclusion Surgical treatment is a safe and effective method in the clinical treatment of hip fracture in the elderly.While repairing bone injury,the incidence of contralateral hip re-fracture after operation is low,which can effectively reduce the interference factors during rehabilitation.
