甲状腺癌患者应用完全腔镜下甲状腺癌根治术与传统开放性手术的疗效对比

Comparison of efficacy between complete endoscopic thyroidectomy and traditional open surgery in thyroid carcinoma patients

目的研究甲状腺癌患者应用完全腔镜下甲状腺癌根治术的临床效果以及与传统开放性手术的疗效。方法 56例甲状腺癌患者,根据手术方法不同分为对照组(30例)和观察组(26例)。对照组患者采用传统开放性手术治疗;观察组患者采用完全腔镜甲状腺癌根治术治疗。比较两组患者围手术期和手术相关并发症的发生情况、手术时间、术中出血量、术后住院时间、负性生存质量评分。结果观察组患者手术时间(98.21±20.07)min、术后住院时间(3.26±1.25)d短于对照组的(138.38±21.71)min、(5.38±1.28)d,术中出血量(23.26±3.12)ml少于对照组的(39.46±3.25)ml,差异具有统计学意义(P<0.05)。观察组患者围手术期和手术相关并发症发生率为11.54%,低于对照组的36.67%,差异具有统计学意义(P<0.05)。随访3、6个月,观察组患者疼痛、失眠、食欲不振及恶心、呕吐评分均低于对照组,差异具有统计学意义(P<0.05)。结论采用完全腔镜下甲状腺癌根治术治疗甲状腺癌效果更好,并发症发生率更低,术后生活质量更高,值得于甲状腺恶性肿瘤临床治疗中借鉴和参考。

Objective To study the efficacy of between complete endoscopic thyroidectomy and traditional open surgery in thyroid carcinoma patients. Methods A total of 56 thyroid carcinoma patients were divided into control group(30 cases) and observation group(26 cases) by different surgical methods. The control group was treated by traditional open surgery, and the observation group was treated by complete endoscopic thyroidectomy. Comparison was made on occurrence of perioperative and operation related complications, operation time, amount of intraoperative hemorrhage, postoperative hospitalization time and negative quality of life score between the two groups. Results The operation time(98.21±20.07) min and postoperative hospitalization time(3.26±1.25) d in the observation group was shorter than(138.38±21.71) min and(5.38±1.28) d in the control group, and amount of intraoperative hemorrhage(23.26±3.12) ml was less than(39.46±3.25) ml in the control group. Their difference was statistically significant(P<0.05). The incidence of perioperative and operation related complications was 11.54% in the observation group, which was lower than 36.67% in the control group, and the difference was statistically significant(P<0.05). After 3 and 6 months of follow-up, the pain, insomnia, anorexia, nausea and vomiting score in the observation group was lower than those in the control group, and their difference was statistically significant(P<0.05). Conclusion Complete endoscopic thyroidectomy shows better therapeutic effect in thyroid carcinoma patients, with lower incidence of complications and higher quality of life after operation. It is worthy of reference in the clinical treatment of thyroid malignant tumors.
