冬病夏治穴位贴敷疗法治疗呼吸系统疾病的流行病学特点调查研究

Epidemiological Characteristics of Acupoint Application Therapy of Winter Disease Cured in Summer on Treatment of Respiratory Diseases

目的:探讨冬病夏治穴位贴敷疗法治疗呼吸系统疾病的流行病学特点。方法:选取2016-2017年于东方医院呼吸热病科冬病夏治穴位贴敷的患者,采用临床流行病学调查问卷的方法分析总结冬病夏治穴位贴敷患者的性别、年龄构成比、呼吸系统疾病分布情况、证候分布特点;同时采用随机对照方法将肺气虚患者分为穴位贴敷组与内外合治组,穴位贴敷组予常规穴位贴敷治疗,内外合治组予穴位贴敷治疗基础上联合口服自拟养肺方,1袋/d,2次/d,疗程为每剂服用3 d,累计9 d,评估内外合治临床疗效。结果:冬病夏治穴位贴敷患者女性多于男性,男女比例为1∶1.49;年龄分布从19岁到99岁,其中60~74岁患者比例最高,占总人数的34.73%;呼吸系统疾病中居前三位的依次为慢性支气管炎(22.42%)、支气管哮喘(20.57%)、慢性咳嗽(18.72%);常见证候中居前三位的依次为肺气虚证(20.71%)、肺脾阳虚证(15.47%)、湿热内蕴证(13.72%);肺气虚贴敷者中内外合治组患者75例,有效率为61.33%,穴位贴敷组患者233例,有效率为46.78%,内外合治组有效率高于穴位贴敷组,差异有统计学意义(P<0.05),内外合治组患者神疲乏力、自汗、畏风、易感冒临床症状改善情况均优于穴位贴敷组,差异有统计学意义(P<0.05)。结论:冬病夏治穴位贴敷疗法在呼吸系统疾病中应用广泛,尤其适用于肺气虚、肺脾阳虚、湿热内蕴证患者,采用内外合治的方法可进一步提高临床疗效,患者依从性好,值得进一步推广。

Objective:To explore the epidemiological characteristics of acupoint application therapy of winter disease cured in summer on the treatment of respiratory diseases. Methods:The patients who were attached to acupoint application therapy of winter disease cured in summer in the Department of Respiratory Diseases of Dongfang Hospital from 2016 to 2017 were selected. The clinical epidemiologicalquestionnaire was used to observe the gender and age ratio of these patients,and to analyze respiratory disease distribution and TCM syndrome distribution characteristics,meanwhile the patients with lung qi deficiency were divided into internal and external combined treatment group and acupoint application group by random control method. Acupoint application group was applied to the conventional acupoint sticking therapy,internal and external combined treatment group was applied to take Yang Fei formula,a bag once,2 times of daily,course of treatment for each take 3 days,the cumulative taking 9 days,then to evaluate internal and external combined treatment clinical curative effect. Results:there were more women than men in the treatment of winter and summer acupoint application,and the ratio of male to female was 1∶1.49.The age distribution ranged from 19 to 99 years,and the proportion of patients aged from 60 to 74 years was the highest,accounting for 34.73%.The top three respiratory diseases were chronic bronchitis(22.42%),bronchial asthma(20.57%)and chronic cough(18.72%).The top three syndromes in common syndromes are lung qi deficiency syndrome(20.71%),lung spleen Yang deficiency syndrome(15.47%),dampheat accumulation syndrome(13.72%).Lung qi deficiency were applied in internal and external combined treatment group,75 cases of patients,effective rate was 61.33%,meridian sticking group,233 cases of patients,efficient 46.78%. Cinical curative effect rate of internal and external combined treatment group was higher than acupoint application group,the difference was statistically significant(P<0.05),patients of internal and external combined treatment group with fatigue,spontaneous perspiration,fear god,easy to catch a cold wind clinical symptoms improved conditions are better than acupoint application group,the difference was statistically significant(P<0.05). Conclusion:Acupoint application of winter disease cured in summer was widely used in respiratory diseases,especially for patients with lung spleen Qi deficiency,lung and spleen Yang deficiency and damp heat internal syndrome. The method of internal and external combined treatment can not only increase the treatment range,but also further improve the clinical efficacy. The patient compliance is good. It is worthy of clinical promotion.
