祛瘀消肿汤加减联合仙灵骨葆胶囊对桡骨远端骨折患者术后生活质量影响

Effect of Quyu Swelling Decoction Modified Combined with Xianling Gubao Capsule on Postoperative Quality of Life in Patients with Distal Radial Fractures

目的:探讨祛瘀消肿汤加减联合仙灵骨葆胶囊对桡骨远端骨折患者术后生活质量的影响。方法:选择我院87例桡骨远端骨折患者,均行常规切开复位内固定手术治疗,按照随机数字表法分组,对照组43例予以仙灵骨葆胶囊治疗,观察组44例予以仙灵骨葆胶囊+祛瘀消肿汤加减治疗,观察比较两组疼痛消失时间、消肿时间、骨折愈合时间、视觉模拟(Visual analogue scale,VAS)评分及腕关节功能恢复、关节活动度变化情况,并统计两组不良反应发生情况及生活质量(The quality of life score,QOLS)评分。结果:观察组消肿、疼痛消失及骨折愈合时间、VAS评分均小于对照组,差异具有统计学意义(P<0.05);观察组腕关节功能恢复优良率为88.64%(39/44),高于对照组68.44%(29/43),差异具有统计学意义(P<0.05);治疗4周后两组患者背伸、掌屈及旋前、旋后活动范围均变大,且观察组较对照组大,差异具有统计学意义(P<0.05);观察组不良反应发生率为9.09%(4/44),对照组为4.65%(2/43),组间比较差异无统计学意义(P>0.05);治疗4周后两组患者QOLS(健康与功能、亲缘与婚姻幸福、个人社会与社区承诺)评分及总分均升高,且观察组较对照组高,差异具有统计学意义(P<0.05)。结论:仙灵骨葆胶囊联合祛瘀消肿汤加减治疗可改善桡骨远端骨折患者术后腕关节功能、关节活动范围及生活质量,缩短骨折愈合及疼痛、肿胀消失时间,且安全性高。

Objective:To evaluate the effect of Quyu Swelling decoction modified combined with Xianling Gubao capsule on postoperative quality of life in patients with distal radius fractures. Methods:78 patients with distal radius fractures in our hospital were randomly divided into control group(n=43)and observation group(n=44),routine open reduction and internal fixation were performed in all patients,the control group was treated with Xianling Gubao capsule,and the observation group were treated with Xianling Gubao capsule + Quyu Swelling decoction modified,pain loss time,swelling time,fracture healing time,visual analogue scale(VAS)score and wrist function recovery,joint mobility changes were observed and compared between the two groups,and statistically analyzed the incidence of adverse events and the quality of life(QOLS)score. Results:The swelling,pain loss and fracture healing time and VAS score in the observation group were lower than those in the control group(P<0.05);The excellent and good rate of wrist function recovery in the observation group 88.64%(39/44)was higher than that in the control group(68.44%,29/43),the difference was statistically significant(P<0.05);Dorsiflexion,paw flexion and pronation,spin range in the observation group were greater than the control group after 2 weeks of treatment,the difference was statistically significant(P<0.05);There was no significant difference in the incidence of adverse reactions between the observation group 9.09%(4/44)and the control group 4.65%(2/43)(P>0.05);Health and function,affinity and marriage happiness,individual social and community commitment and QOLS score in the observation group were higher than the control group after 2 weeks of treatment,the difference was statistically significant(P<0.05). Conclusion:Treatment of distal radius fractures patients using Quyu Swelling decoction modified combined with Xianling Gubao capsule can improve the postoperative wrist function,joint activity range and quality of life,shorten the fracture healing and pain,swelling disappear time,with high safety.
