自拟祛痛膏外敷联合针刺疗法治疗腰椎间盘突出症临床研究

Clinical Study of Self-Made Black Plaster Combined with Acupuncture in Treatment of Lumbar Disc Herniation

目的:通过对腰椎间盘突出症患者进行自拟祛痛膏外敷联合针刺疗法治疗,观察其临床疗效。方法:选取2017年7月-2018年7月本院住院的腰椎间盘突出症患者78例,所有病例随机平分为A组(自拟祛痛膏外敷联合针刺疗法治疗) 42例,B组(单纯采用针刺疗法) 36例。观察比较两组治疗前后视觉模拟评分(VAS)、JOA评分以及SC-ODI评分,并行组内与组间比较。两组根据中医病证诊断疗效标准进行疗效评估。结果:两组治疗后及随访3、6个月疼痛VAS评分及SC-ODI评分均较本组治疗前降低(P <0. 05),JOA评分均较本组治疗前升高(P <0. 05);A组治疗后及随访3、6个月疼痛VAS评分及SC-ODI评分均低于B组同期(P <0. 05),JOA评分高于B组同期(P <0. 05)。A组临床治愈率69%,B组临床治愈率38. 9%,A组临床治愈率明显高于B组(P <0. 05)。结论:自拟祛痛膏外敷联合针刺疗法治疗腰椎间盘突出症,可显著提高患者临床疗效,减轻疼痛,提高患者的腰椎功能,并降低腰痛对患者日常生活的影响,优于单纯的针刺治疗。

Objective: To observe the clinical efficacy of external application of acupuncture and moxibustion in patients with lumbar disc herniation. Methods: Seventy-eight patients with lumbar disc herniation admitted to our hospital from July 2017 to July 2018 were enrolled. All patients were randomly divided into group A( 42 cases were treated with self-made pain-relieving cream for external application combined with acupuncture treatment) and group B( 36 cases were treated with acupuncture alone).The visual analogue scale( VAS),JOA score and SC-ODI score before and after treatment were compared between the two groups. The two groups were evaluated for efficacy based on the diagnostic criteria of TCM syndromes. Results: The pain VAS score and SC-ODI score of the two groups after treatment and 3 and 6 months after follow-up were lower than those before treatment( P < 0. 05),and the JOA scores were higher than those before treatment( P < 0. 05). The pain VAS score and SC-ODI score of group A after treatment and 3 and 6 months after follow-up were lower than those of group B( P < 0. 05),and JOA score was higher than that of group B( P < 0. 05). The clinical cure rate was 69% in group A and 38. 9% in group B. The clinical cure rate in group A was significantly higher than that in group B( P < 0. 05). Conclusion: The application of acupuncture combined with self-made pain-relieving cream for external application can significantly improve the clinical efficacy,reduce pain,improve the lumbar function of patients and reduce the impact of low back pain on patients’ daily life,which is better than simple acupuncture. treatment.
