急性病毒性心肌炎的临床特点及影响因素分析

Clinical characteristics and influencing factors for acute viral myocarditis

目的探究急性病毒性心肌炎的临床特点及与其影响因素。方法选取河南省胸科医院收治的病毒性心肌炎患者84例,其中急性病毒性心肌炎组20例,非急性病毒性心肌炎组64例,统计急性病毒性心肌炎患者的病情持续时间及临床表现,比较分析两组患者的超声心动图、心电图表现及血液生化指标。结果急性病毒性心肌炎患者左心室射血分数(LVEF)低于非急性病毒性心肌炎患者。急性病毒性心肌炎患者N末端B型利钠肽原(NT-proBNP)高于非急性病毒性心肌炎患者,差异具有统计学意义(P<0.05)。急性病毒性心肌炎患者PR间期及QRS波间期均高于非急性病毒性心肌炎患者,PR间期>200 ms率及QRS间期>120 ms率也都高于非急性病毒性心肌炎患者,差异具有统计学意义(P<0.05)。结论急性病毒性心肌炎患者临床表现为晕厥,发热及心源性休克等症状,QRS间期延长与PR间期延长是急性病毒性心肌炎的影响因素。

OBJECTIVE To explore the clinical characteristics and influencing factors for acute viral myocarditis.METHODS A total of 84 patients with viral myocarditis who were treated in Henan Thoracic Hospital were enrolled in the study and divided into the acute viral myocarditis group with 20 cases and the non-acute viral myocarditis group with 64 cases.The duration of illness condition and clinical manifestations of the patients with acute viral myocarditis were statistically analyzed.The echocardiogram,electrocardiogram manifestations and blood biochemical parameters were observed and compared between the two groups of patients.RESULTS The left ventricular ejection fraction(LVEF)of the acute viral myocarditis patients was significantly lower than that of the non-acute viral myocarditis patients,the N-terminal B-type natriuretic peptide(NT-proBNP)of the acute viral myocarditis patients was significantly higher than that of the non-acute viral myocarditis patients significantly(P<0.05).The PR interval and QRS interval of the acute viral myocarditis patients were significantly higher than those of the non-acute viral myocarditis patients;the rates of PR interval more than 200 ms and QRS interval more than 120 ms of the acute viral myocarditis patients were significantly higher than those of the non-acute viral myocarditis patients(P<0.05).CONCLUSION Syncope,fever and cardiogenic shock are the clinical manifestations of the patients with acute viral myocarditis;the prolonged QRS and PR intervals are the influencing factors for the acute viral myocarditis.
