HPV感染合并子宫内膜癌对人附睾蛋白4和 Th1/Th2细胞因子的影响

Influence of HPV infection complicated with endometrial cancer on human epididymis protein 4 and Th1/Th2 cytokines

目的探讨人乳头瘤病毒(HPV)感染合并子宫内膜癌对人附睾蛋白4(HE4)、辅助性T细胞1/辅助性T细胞2(Th1/Th2)细胞因子的影响。方法以天津市第五中心医院2015年1月-2018年6月收治的77例HPV感染合并子宫内膜癌患者为研究组,选择77例子宫内膜癌患者为对照组,比较两组患者血清HE4、外周血T淋巴细胞(CD4+、CD8+、CD4+/CD8+)及相关细胞因子[干扰素-γ(INF-γ)、白细胞介素-2(IL-2)、IL-4、IL-10]水平。结果与对照组比较,研究组血清HE4、CD8+、IL-4、IL-10水平更高,而CD4+、INF-γ、CD4+/CD8+水平更低(P<0.05)。研究组中,不同病理类型、分级以及HPV分型患者血清HE4及Th1/Th2细胞因子水平比较无统计学差异。而有淋巴结转移及浸润深度≥1/2的患者血清IL-4、IL-10水平更高(P<0.05)。结论HPV感染合并子宫内膜癌HE4呈过表达,并存在一定程度的免疫抑制,而Thl向Th2偏移可能是HPV感染合并子宫内膜癌发生发展的原因之一。

OBJECTIVE To explore the influence of human papillomavirus(HPV)infection complicated with endometrial cancer on human epididymis protein 4(HE4)and helper T cell 1/helper T cell 2(Th1/Th2)cytokines.METHODS Totally 77 HPV infection patients who were complicated with endometrial cancer and treated in The Fifth Central Hospital of Tianjin from Jan 2015 to Jun 2018 were assigned as the study group,and 77 patients who had endometrial cancer were set as the control group.The levels of serum HE4,peripheral blood T lymphocytes(CD4+,CD8+,CD4+/CD8+)and cytokines[interferon-γ(INF-γ),interleukin-2(IL-2),IL-4,IL-10]were observed and compared between the two groups of patients.RESULTS The levels of serum HE4,CD8+,IL-4 and IL-10 of the study group were significantly higher than those of the control group,while the levels of CD4+,INF-γand CD4+/CD8+of the study group were significantly lower than those of the control group(P<0.05).There was no significant difference in level of serum HE4 or Th1/Th2 cytokine among the patients with different pathological patterns,grades or HPV genotypes.The levels of serum IL-4 and IL-10 of the patients who had lymphatic metastasis and were with the depth of invasion no less than 1/2 were higher(P<0.05).CONCLUSION The HE4 shows over-expressed in the HPV infection patients complicated with endometrial cancer,who has a certain degree of immunosuppression.The shift of Th1 to Th2 may be one of the causes of occurrence and progression of HPV infection complicated with endometrial cancer.
