139例儿童慢性肺炎中医证型分布及用药规律探讨

Exploration of the Distribution Characteristics and Medication Rules of Clinical Syndromes in 139 Cases of Children with Chronic Pneumonia

目的:探讨儿童慢性肺炎临床证型分布特点与用药规律。方法:收集符合纳入条件儿童慢性肺炎患儿139例,以证型分布及中药处方为研究点,使用SPSS20. 0统计学软件探讨儿童慢性肺炎证型分布特点及中医用药规律。结果:儿童慢性肺炎证型分布首位为肺脾气虚证,与其他证型比较差异有统计学意义。用药方面太子参、炒白术、紫菀、款冬花、橘红、橘络、桃仁、红花为常用药物,类别方面止咳化痰平喘类药物应用最多,补虚药、理气药、清热药、活血化瘀药次之。结论:肺脾气虚是儿童慢性肺炎的主要中医证型,治疗上中药止咳化痰药、补虚药、理气药、清热药及活血化瘀药运用较多,体现了中医治则应以补肺健脾、止咳平喘为主,在此基础上加用理气、清热、化瘀之药的用药规律。

Objective:To explore the distribution characteristics and medication rules of clinical syndromes in children with chronic pneumonia.Methods:139 children with chronic pneumonia children were collected according to the inclusion criteria,the syndrome distribution and prescription of Traditional Chinese Medicine(TCM)were regarded as the research points.All the data were analyzed with SPSS 20.0 statistics software.Results:The leading pattern of TCM syndrome of children chronic pneumonia was the syndrome of Qi deficiency in lung and spleen,and there was a significant difference when compared with other types.Herbs most frequently used were Taizishen,Chaobaizhu,Ziyuan,Kuandonghua,Juhong,Juluo,Taoren,Honghua.Regarding to the classification of herbs,the top two types that most widely prescribed were those who can relieve cough,reduce sputum and asthma and restore vital energy,others such as Qi-regulating drugs,heat-clearing drugs,blood-activating and stasis-eliminating drugs were also used constantly.Conclusion The syndrome of Qi deficiency of lung and spleen is the main TCM syndrome type of chronic pneumonia in children,drugs which can relieve cough,reduce sputum and asthma and restore vital energy,along with those can regulate Qi,clear heat,activate blood and eliminate stasis were most widely used,suggesting that main principals of TCM treatment in chronic pneumonia are tonifying lung and spleen and reducing cough and asthma,on the basis of which,drugs aimed to regulate Qi,clear heat,or eliminate stasis were used as a supplement.
