脐灸联合三阶梯止痛法治疗虚寒型癌痛临床观察

Clinical Observation on Umbilical Moxibustion Combined with Three-step Analgesia in Patients with Deficiency and Cold Cancer Pain

目的 探讨脐灸结合三阶梯止痛法对虚寒型癌痛患者镇痛效果及安全性的影响。方法 选择2016年10月-2018年10月青岛市西海岸新区第二中医医院治疗的40例虚寒型癌痛患者,将其应用随机数字表法分为2组,均20例。对照组接受三阶梯止痛法治疗,观察组接受脐灸、三阶梯止痛法治疗。比较2组镇痛效果、镇痛剂使用剂量及不良反应。结果 观察组起效时间(20. 65±6. 58) min短于对照组的(41. 57±9. 45) min,维持时间(8. 97±0. 93) h长于对照组的(6. 57±0. 78) h,氨酚双氢可待因片、硫酸吗啡缓释片使用剂量(80. 65±21. 36) mg、(81. 57±33. 45) mg低于对照组的(151. 57±64. 56) mg、(114. 57±40. 57) mg,恶心呕吐、头痛头晕、便秘、排尿困难发生率分别为20. 00%、10. 00%、25. 00%,5. 00%均低于对照组的55. 00%、40. 00%、60. 00%、35. 00%,差异有统计学意义(P <0. 05)。结论 虚寒型癌痛患者接受脐灸、三阶梯止痛法治疗,镇痛效果确切且不良反应少,利于降低镇痛药使用剂量,减轻患者生理痛苦。

Objective To investigate the effect of umbilical moxibustion combined with three-step analgesia on analgesic effect and safety in patients with deficiency cold cancer pain.Methods From October 2016 to October 2018,40 patients with deficiency cold cancer pain treated in the Second Hospital of Traditional Chinese Medicine of West Coast New District of Qingdao were randomly divided into two groups,with 20 cases in each group.The control group was treated with three-step analgesia,while the observation group was treated with umbilical moxibustion and three-step analgesia.The analgesic effect,dosage and side effects of analgesic agents were compared between the two groups.Results The onset time of the observation group(20.65±6.58)min was shorter than that of the control group(41.57±9.45)min.The maintenance time(8.97±0.93)h was longer than that of the control group(6.57±0.78)h.The incidence of urination,vomiting,headache dizziness,constipation and dysuria was 20.00%and 10.00%,respectively,when the dosage of paracetamol dihydrocodeine tablets and morphine sulfate sustained-release tablets was(80.65±21.36)mg,(31.57±33.45)mg,which was significantly lower than that of the control group(151.57±64.56)mg,(114.57±40.57),and the incidence of nausea vomiting,headache dizziness,constipation and dysuria was 20.00%and 10.00%,respectively.25.00%,5.00%were lower than those in the control group 55.00%,40.00%,60.00%,35.00%,and the difference was statistically significant(P<0.05).Conclusion Umbilical moxibustion combined with three-step analgesia on analgesic effect and safety in patients with deficiency cold cancer pain has definite analgesic effect and less adverse reactions,which is beneficial to reduce the dosage of analgesic drugs and alleviate the physiological pain of patients.
