摘要
目的:探究自我管理联合心理引导对支气管哮喘患者情绪调节自我效能感的影响。方法:将我院2016年4月-2017年7月收治的74例支气管哮喘患者按照随机数表法分为观察组与对照组各37例,对照组在支气管哮喘规范化治疗基础上联合心理引导,观察组在对照组基础上联合自我管理教育,持续干预6个月后,比较两组焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、自我管理行为评分、情绪调节自我效能感量表(SRESE)评分、肺功能指标[一秒用力呼吸量(FEV1)、用力呼气量占用力肺活量比值(FEV1/FVC)、最大呼气峰流速(PEF)]、成人哮喘生命质量量表(AQLQ)评分。结果:观察组干预后SDS评分、SAS评分显著低于对照组,自我管理行为评分、SRESE评分显著高于对照组,FEV1、FEV1/FVC、PEF指标改善情况显著优于对照组,活动受限、哮喘症状、心理状况、刺激源反应、对自身健康关心各项AQLQ评分显著高于对照组,以上差异均有统计学意义(t=-6.628,-5.715,6.687,6.561,2.062,2.105,2.116,3.200,5.892,4.793,4.603,6.033;P<0.05)。结论:支气管哮喘患者应用自我管理联合心理引导,可显著改善肺功能损伤,提高情绪调节自我效能感及生活质量。
Objective:To explore the effects of self-management combined with psychological guidance on emotion-regulated self-efficacy in patients with bronchial asthma.Methods:A total of 74 cases of patients with bronchial asthma admitted to our hospital from April 2016 to July 2017 were divided into the observation group(n=37)and the control group(n=37)according to the random number table method.The control group was given the psychological guidance on the basis of bronchial asthma standardization,and the observation group was given the self-management education on the basis of the control group.After 6 months of continuous intervention,the self-rating anxiety scale(SAS)score,self-rating depression scale(SDS)score,self-management behavior score,emotion-regulated self-efficacy scale(SRESE)score,pulmonary function indexes[forced expiratory volume in one second(FEV1),the radio of forced expiratory volume in forced vital capacity(FEV1/FVC),peak expiratory flow(PEF)]and the adult asthma quality of life questionnaire(AQLQ)score were compared between the two groups.Results:After intervention,the scores of SDS and SAS in the observation group were significantly lower than those in the control group,and the self-management behavior score and SRESE score were significantly higher than those in the control group,and the improvement of FEV1,FEV1/FVC and PEF indexes was significantly better than that in the control group,and the AQLQ scores of activity limitation,asthma symptoms,psychological status,stimulus response and the concern for their own health were significantly higher than those in the control group,and the above differences were statistically significant(t=-6.628,-5.715,6.687,6.561,2.062,2.105,2.116,3.200,5.892,4.793,4.603,6.033;P<0.05).Conclusion:The self-management combined with psychological guidance in patients with bronchial asthma can significantly improve the lung function injuries and enhance the emotion-regulated self-efficacy and quality of life,therefore it is worthy of wide recommendation.
