肺表面活性物质联合CPAP治疗对新生儿呼吸窘迫综合征血气指标的作用分析 被引量:3

摘要 目的观察新生儿呼吸窘迫综合征临床治疗中应用持续正压通气(CPAP)联合肺表面活性物质治疗的临床效果,并分析其对患儿血气指标的影响。方法选取2016年11月~2018年11月在本院新生儿科接受住院治疗的72例新生儿呼吸窘迫综合征作为研究对象。入院后患儿均接受常规基础治疗,按照随机抽签的方法将其分为两组,其中36例给予持续正压通气治疗(常规治疗组),其余36例给予持续正压通气联合肺表面活性物质治疗。结果检测结果显示出不同治疗方案均在改善患儿动脉血气状态方面获得明显效果,组内治疗前后比较各动脉血气指标均得到有效改善,P<0.05。但联合治疗组的改善效果明显优于常规治疗组,组间比较治疗后两组相关指标,差异显著,联合治疗组指标检测结果明显优于常规治疗组,P<0.05。在平均住院时间方面,常规治疗组为(22.74±1.38)d,联合治疗组为(16.35±1.27)d,差异有统计学意义(P<0.05)。结论新生儿呼吸窘迫综合征临床治疗中应用持续正压通气联合肺表面活性物质治疗可获得理想的疗效,显著改善患儿的血气指标,可在临床进行一定的推广应用。 Objective To observe clinical curative effects of CPAP(continuous positive airway pressure) combined with pulmonary surfactant in clinical treatment of neonatal respiratory distress syndrome and analyze its effect on blood gas index. Methods 72 neonates with neonatal respiratory distress syndrome were selected as research objects and they were hospitalized in the department of neonatology in our hospital from November 2016 to November 2018.After admission,neonates underwent routine basic treatment.According to the random lottery method,they were divided into two groups.36 cases were treated with continuous positive airway pressure(routine treatment group) and the remaining 36 cases were treated with continuous positive airway pressure combined with pulmonary surfactant. Results The results showed that different treatment regimens had significant effects in improving the arterial blood gas status of the neonates.Every arterial blood gas index after treatment was effectively improved compared with that before treatment,P<0.05.However,the improvement effect of the combined treatment group was significantly better than the conventional treatment group.There were significant differences in the correlation indexes between the two groups after treatment. The results of the combined treatment group were significantly better than the conventional treatment group,P<0.05.In terms of mean hospital stay,that of the conventional treatment group was(22.74±1.38) days and that of the combined treatment group was(16.35±1.27) days.There was statistically significant difference in data of two groups(P<0.05). Conclusion Continuous positive airway pressure combined with pulmonary surfactant in clinical treatment of neonatal respiratory distress syndrome can obtain ideal curative effects.It can significantly improve the blood gas index of neonates,which can be widely prompted and applied in clinical practice.
