双能X线骨密度仪评估绝经期妇女体脂含量对骨密度的影响分析 被引量:3

摘要 目的探讨双能X线骨密度仪评估绝经期妇女体脂含量对骨密度的影响。方法选取2017年1月~2019年5月在我院就诊的绝经期妇女102例,所有目标人群按照体脂分组,分为低体脂组,中等体脂组,高体脂组,各34例。采用美国的hologic型双能X线骨密度仪测定骨密度、BMD、体脂含量等,主要采集部位为非优势侧股骨颈、腰椎(包括L1、L2、L3、L4)骨密度及体内BMD、总体脂肪质量、总体肌肉质量及区域脂肪、肌肉质量等指标。结果三组的年龄,身高等比较,差异无统计学意义(P>0.05);腰围,颈围,上臂围比较,差异有统计学意义(P<0.05);且高体脂组显著高于低体脂组及中等体脂组(P<0.05)。三组总体脂肪质量、总体肌肉质量、上肢、大腿、躯干脂肪质量、大腿脂肪质量/总体脂肪质量、躯干脂肪质量/总体脂肪质量比较,差异有统计学意义(P<0.05);且高体脂组显著高于低体脂组及中等体脂组(P<0.05)。高体脂组骨密度高于中等体脂组及低体脂组,差异有统计学意义(P<0.05);三组BMD、股骨颈、全髋、腰椎骨密度比,差异均有统计学意义(P<0.05)。脂肪组织含量和DXA骨密度值呈正相关(P<0.05)。结论双能X线骨密度仪可评估绝经期妇女体脂含量,绝经期妇女体脂含量与骨密度呈正相关。 Objective To discuss Effect of body fat content on bone mineral density in menopausal women assessed by dual-energy X-ray absorptiometry. Methods One hundred and two menopausal women in our hospital from January 2017 to May 2019 were selected. All the target groups were divided into low body fat group, medium body fat group and high body fat group, 34 cases each. Bone mineral density, BMD and body fat content were measured by hologic dual-energy X-ray absorptiometry in the United States. Bone mineral density of non-dominant femoral neck, lumbar spine(including L1, L2, L3, L4) and BMD, total fat mass, total muscle mass, regional fat and muscle mass were collected. Results There was no significant difference in age and height between the three groups(P>0.05), there was significant difference in waist circumference, neck circumference and upper arm circumference(P<0.05), and the high body fat group was significantly higher than the low body fat group and the middle body fat group(P<0.05). There were significant differences in total fat mass, total muscle mass, upper limb, thigh, trunk fat mass, thigh fat mass/total fat mass, trunk fat mass/total fat mass among the three groups(P<0.05), and the high body fat group was significantly higher than the low body fat group and the middle body fat group(P<0.05). The BMD of the high body fat group was higher than that of the medium body fat group and the low body fat group(P<0.05), and the BMD, femoral neck, total hip and lumbar vertebrae of the three groups were significantly different(P<0.05). Adipose tissue content was positively correlated with DXA bone mineral density(P<0.05). Conclusion Dual-energy X-ray absorptiometry can evaluate the body fat content of menopausal women. The body fat content of menopausal women is positively correlated with bone mineral density.
