美学区单颗牙即刻种植即刻修复的美学效果

Aesthetic Effect of Immediate Implantation and Immediate Restoration of Single Tooth in Aesthetic Area

目的讨论单颗牙即刻种植即刻修复在美学区的临床应用,并评价其效果。方法选取2016年1月-2018年12月我院12例前牙单颗牙唇侧骨板完整的患者。48小时内将12枚种植体植入临时基台,用纵向螺钉固定,即刻修复树脂临时冠,咬合无接触。6~8个月后,利用个性化转移杆技术,将种植体周围软组织的形态精确转移到石膏模型上,选择氧化锆全瓷冠作为永久性修复体。结果追踪9~13个月,种植体骨融合良好,无种植体丢失,观察即刻种植即刻诱导牙龈成形的"红色美学"效果,与天然牙协调一致,牙冠的"白色美学"效果评分也较高,患者对即刻种植即刻修复效果满意。结论严格控制即刻种植即刻修复适应症的前提下,美学区即刻种植即刻修复,具有治疗周期短,美观效果好的优点,近期红白美学效果较好,长期效果有待进一步追踪。

Objective To discuss the clinical application of single tooth immediate implantation and immediate restoration in aesthetic area and evaluate its effect.Methods A total of 12 patients with complete labial bone plate of a single anterior tooth in our hospital from January 2016 to December 2018 were selected.Within 48 hours,12 implants were implanted into the temporary abutment,fixed with longitudinal screws,and the resin temporary crown was immediately repaired with no contact in occlusion.After 6~8 months,the soft tissue morphology around the implant was accurately transferred to the plaster model by using personalized transfer rod technology,and the zirconia all-porcelain crown was selected as the permanent prosthesis.Results After 9~13 months of follow-up,the implant bone fusion was good and no implant loss was observed.The"red aesthetic"effect of immediate implantation and immediate induction of gingival formation was observed,which was in harmony with the natural teeth.The"white aesthetic"effect score of the crowns was also high,and the patients were satisfied with the immediate implant and immediate restoration effect.Conclusion Under the premise of strict control of indications for immediate planting and immediate repair,immediate planting and immediate repair in aesthetic area has the advantages of short treatment period and good aesthetic effect.The short-term red and white aesthetic effect is better,and the long-term effect needs to be further tracked.
