彩色多普勒超声在股骨颈骨折术前深静脉血栓筛查中的应用

Application of Color Doppler Ultrasonography in the Screening of Deep Vein Thrombosis Before Femoral Neck Fracture

目的探究彩色多普勒超声在股骨颈骨折术前深静脉血栓筛查中的应用。方法选择2017年2月—2018年12月天津市天津医院接收的股骨颈骨折患者600例作为研究对象,利用彩色多普勒超声诊断仪对患者进行术前深静脉血栓检测,将血管造影介入治疗系统检查的结果作为金标准。对比两种检查的结果,分析总结彩色多普勒超声检查确诊率、敏感性、特异性以及准确性。结果血管造影介入治疗系统检查结果发现,存在血栓患者139例,无血栓患者461例,确诊率23.17%;彩色多普勒超声检查中,存在血栓患者141例,无血栓患者459例,确诊率23.50%,差异无统计学意义(χ2=0.019,P=0.891);彩色多普勒超声检查中有6例误诊,2例漏诊,误诊、漏诊率为1.33%。彩色多普勒超声诊断深静脉血栓的敏感性为95.68%,特异性为98.26%,准确性为97.67%。结论彩色多普勒超声在股骨颈骨折术前深静脉血栓筛查中诊断率较高,误诊、漏诊率较低,同时诊断敏感性、特异性以及准确性较高,可靠性较高,运用价值较高,其检测结果可作为深静脉血栓筛查的依据。

Objective To investigate the application of color Doppler ultrasound in the screening of deep vein thrombosis before femoral neck fracture.Methods A total of 600 patients with femoral neck fractures received from the hospital from February,2017 to December 2018 were enrolled in the study.Color Doppler ultrasound was used to detect preoperative deep vein thrombosis and angiographic interventional system was performed.The result was the gold standard.Through comparing the results of the two examinations,the analysis summarized the diagnostic rate,sensitivity,specificity and accuracy of color Doppler ultrasonography.Results The results of angiographic interventional system showed that there were 139 patients with thrombosis and 461 patients without thrombosis.The diagnosis rate was 23.17%.In color Doppler ultrasonography,there were 141 patients with thrombosis and 459 patients without thrombosis.The diagnosis rate was 23.50%The difference was not statistically significant(χ2=0.019,P=0.891);6 cases were misdiagnosed in color Doppler ultrasonography,2 cases were missed diagnosis,and misdiagnosis and missed diagnosis rate was 1.33%.The sensitivity of color Doppler ultrasound in the diagnosis of deep vein thrombosis was 95.68%,specificity was 98.26%,and accuracy was 98.67%.Conclusion Color Doppler ultrasound has a higher diagnostic rate in the diagnosis of deep venous thrombosis before femoral neck fracture.The rate of misdiagnosis and missed diagnosis is low.The sensitivity,specificity and accuracy of diagnosis are high,reliability is high,and the application value is high.The test results can be used as a basis for deep vein thrombosis screening.
