义乌市中医医院2015—2018年429例药品不良反应分析

Analysis of 429 cases of adverse reactions in Yiwu Traditional Chinese Medicine Hospital from 2015 to 2018

目的 探讨义乌市中医医院2015—2018年429例不良反应情况,为临床合理用药提供依据。方法 选择2015年1月至2018年12月义乌市中医医院药品不良反应患者429例作为对象,查阅患者病案资料,分析药品不良反应患者药物类型、科室分布;对于确诊的药品不良反应患者积极采取有效的措施进行治疗干预,记录药品不良反应事件的结果,针对药品不良反应产生的原因制定有效的措施进行干预。结果 429例药品不良反应患者中发生率年份排在前两位的分别为2018年与2017年,分别占29.14%与26.81%;429例药品不良反应中药物类型相对较多,排在前3位的分别为抗菌药物、心血管系统用药、肠道外营养用药,分别占33.33%、26.11%和14.92%;义乌市中医医院429例药品不良反应患者分布科室相对较广,科室分布前3位分别为急诊科、内科和骨伤科,分别占27.04%、24.94%和17.25%;义乌市中医院429例药品不良反应患者确诊后结合患者症状给予对应治疗,患者症状均得到明显改善,治愈及好转率为100.00%。结论 义乌市中医医院2015—2018年药品不良反应发生率较高,且以抗菌药物为主,主要分布在急诊科、内科和骨伤科中,应加强临床药物干预、指导,提高用药安全性。

Objective To investigate the adverse reactions of 429 cases in Yiwu Traditional Chinese Medicine Hospital from 2015 to 2018,and provide evidence for clinical rational drug use.Methods Totally 429 patients with adverse drug reaction(ADR)in Yiwu Traditional Chinese Medicine Hospital from January to February 2018 were selected as subjects.The patient case data were reviewed to analyze the drug type and department distribution of ADR patients.Patients diagnosed with ADR were carried out effective measures for treatment intervention,the results of ADR were recorded,and effective measures for intervention were formulated according to the causes of ADR.Results Among the 429 patients with ADR,the top two were in 2018 and 2017,with 29.14%and 26.81%respectively;429 patients with ADR had relatively more drug types,and ranking the top three were antibacterial drugs,cardiovascular system drugs,parenteral nutrition drugs,accounting for 33.33%,26.11%and 14.92%respectively;429 cases of ADR patients in Yiwu Traditional Chinese Medicine Hospital were relatively wide,and the top three departments were the Emergency Department,the Internal Medicine Department,and the Orthopedics Department,accounting for 27.04%,24.94%and 17.25%respectively;429 cases of ADR patients in Yiwu Traditional Chinese Medicine Hospital were treated with corresponding symptoms after diagnosis.The symptoms of the patients were significantly improved,and the cure and improvement rate was 100.00%.Conclusion The incidence of ADR in Yiwu Traditional Chinese Medicine Hospital from 2015 to 2018 is high,and it is mainly based on antibiotics,mainly in the Emergency Department,the Internal Medicine Department and the Orthopedics Department.It is necessary to strengthen clinical drug intervention and guidance to improve the safety of medication.
