某院超长住院日患者的分析与对策

Analysis and countermeasures of patients with overlong inpatient days in a hospital

目的 分析某医院161例住院天数超过50天出院患者的分布特征,为医院管理部门有效缩短平均住院日提供有效依据。方法 将住院日超过 2018 年出院患者住院日的99%分位数( P99=50天)定义为超长住院日。采用Excel对数据进行分析。结果 2018年全院出院患者11 465人次,平均住院日9.31天,主要集中年龄段为40~49岁。超长住院日患者男女性别比2.10 ∶1,平均费用为83 249.20元,科室分布主要集中在关节外科、皮瓣外科、内科,疾病排名前三的为损伤、中毒和外因的某些其他后果,循环系统疾病、肌肉骨骼系统和结缔组织疾病。结论 加强科室管理,实施临床路径管理,改进门诊服务模式,加强医患沟通。

Objective To analyze the distribution characteristics of 161 inpatients discharged after more than 50 days of hospitalization from a hospital,and to provide an effective basis for the hospital management to shorten the average length of stay.Methods The hospital stay exceeding 99%quantile(P 99=50 days)of the hospital stay of discharged patients in 2018 was defined as the extremely long hospital stay.Excel was used to analyze the data.Results In 2018,11465 patients were discharged from the hospital,with an average hospitalization time of 9.31 days and the main age group was from 40 to 49 years old.The sex ratio of men and women patients with overlong hospitalization was 2.10∶1 and the average cost was 83249.20 yuan,The department distribution mainly concentrated in Joint Surgery,Flap Surgery,and Internal Medicine,and the top three diseases were injury,poisoning and certain other consequences of external causes,diseases of the circulatory system,and diseases of the musculoskeletal system and connective tissues.Conclusion We should strengthen the management of departments,implement the clinical pathway management,improve the outpatient service model,and strengthen the doctor-patient communication.
